Dear Ms Karoly,
We received your letter of May 10, 2007 advising us that the application package
we submitted on May 9, 2007 was incomplete as it did not include the permit fee.
I did not include the permit fee as I had been told by Amy Chapman that no fee
was required for this application since it was afollow-up to an earlier application
(DWQ #06-1965) for which we had paid the required fee. Nevertheless, please
find enclosed my check in the amount of $200 as payment for the permit fee. If
you should determine that the fee is not required, feel free to destroy the check
rather than go through the trouble of returning it -just let me know about it.
