Cause of spill/bypass: Contractors had left a inflatable line plug in the new sewer line and had not removed it before allowing customers to use the line. 3. Did you have personnel available to perform initial assessment 24 hours/day (including weekends and holidays)? Yes X No 4. Hours 15 Minutes Please explain the time taken to make initial assessment: locating the exact area of the spill and then start removing the blockage. The work crew was already on site.
