2. Cause of spill/bypass: Contractors had left a inflatable line plug in the new sewer line and had not
removed it before allowing customers to use the line.
3. Did you have personnel available to perform initial assessment 24 hours/day (including weekends and
holidays)?

Please explain the time taken to make initial assessment: locating the exact area of the spill and then
start removing the blockage. The work crew was already on site.

5. Action taken to contain spill, clean up waste, and/or remediate the site: once the obstruction was
removed from the line the area was cleaned and all debris removed and the area was limed.
