Dear Permittee:
The North Carolina Division of Water Resources conducted an inspection of the Mountain View Assisted
Living on 2/19/2021. This inspection was conducted to verify that the facility is operating in compliance
with the conditions and limitations specified in NPDES WW Permit No. NC0074110. The findings and
comments noted during this inspection are provided in the enclosed copy of the inspection report
entitled "Compliance Inspection Report".
There were no significant issues or findings noted during the inspection and therefore, a response to this
inspection report is not required.

On 2/19/2021, I (Daniel Boss) and Stephanie Williams conducted a compliance evaluation inspection
of the Mountain View Assisted Living WWTP. We met with Mark Jones (ORC), Daniel Jones, and
Henry (Mizpah groundskeeper) for the inspection.
Overall the WWTP appeared to be operating well. The effluent was clear, settling in the clarifier was
adequate, and the tertiary filters look well maintained.
The crushed effluent line has been repaired and the chlorine contact chamber is no longer backing up.

The clarifier blanket was measured at 2.5' out of 7.5' depth. There was approximately 4' of
clear wastewater from the surface.

Settleability was 30% at 25 minutes. There was some pinfloc floating in the supernatant, but
the clarifier had 4' of clear wastewater, so the presence of pinfloc is not concerning.
