Enclosed please find a copy of the Compliance Evaluation Inspection form from the inspection conducted on
8/20/2014. The Compliance Evaluation Inspection was conducted by Mark Brantley, Environmental Senior
Specialist, of the Fayetteville Regional Office. The cooperation of Mr. Donald Allbright, facility ORC, was greatly
appreciated. The facility was found to be in Compliance with permit NC0003719. As a reminder, preservation of
the Waters of the State can only be achieved through consistent NPDES Permit compliance.
Comments
• The effluent from the WWTP was clear and free of visible solids at the time of the inspection.
Maintenance records are maintained through a work order system. The ORC and laboratory records were
in order.

The report was mailed to the address the Division has on record (P. O. Box 1690, Fayetteville,.
NC, 28302) and not the current physical address. The US Postal Service returned the. letter to the
Fayetteville Regional Office on January 26, 2015 as not deliverable as addressed. The Division
apologizes for any inconvenience that this may have caused.
