On June 6, 2012, DWQ requested more information so that we may process this request. On July 20, 2012, you submitted a buffer authorization application dated July 19, 2012 to fill or otherwise impact 980 square feet of Zone 1 Neuse River protected riparian buffer and 631 square feet of Zone 2 Neuse River protected riparian buffer associated with the redesign of these level spreaders. DWQ's Request for More Information Letter dated June 6, 2012 stated: "You provided the Level Spreader Supplement Form for each proposed level spreader, but failed to fill out Part III: Required Items Checklist. Please fill out the Required Items checklist and be certain to provide ALL items at the requested scales. If you fail to provide these items correctly in your next submittal, the project will be returned." The submittal received by DWQ on July 20, 2012 did not include completed Required Items checklists nor did it include all of the required items at the correct scales. For this reason your permit application is hereby considered returned. Once you have redesigned your project to address the problems and inadequacies with your application as it was submitted, you will need to reapply for DWQ approval. This includes submitting a complete application package with the appropriate fee. Please note that the review of the July 20, 2012 submittal also revealed, at a minimum, the following deficiencies: * An agent authorization letter was not included authorizing the engineer signatory authority of the application; The bypass channels appeared to impact wetlands but a 401 Water Quality Certification (WQC) was not requested nor has a 401 WQC been secured for this activity.
