Dear Chad:
Enclosed is a 401 Water Quality Certification (3883) application for your review. This
application is submitted on behalf of Security Saving Bank c/o Mr. Henry Edmund for after -the-
fact authorization of a road widening and the repair of an earthen dam located within the
Rutledge Subdivision in Shallotte, NC (Brunswick County). The road widening has impacted
approximately 0.08 acre of wetlands and 20 linear feet of stream.- The earthen dam repair will
impact 0.004 acre of wetlands (0.002 ac permanent and 0.002 ac teporary).
As you are aware, the previous landowner received letters of non - compliance from the
US Army Corps of Engineers and the NC Division of Water Quality for this project. Since that
time, Security Savings Bank has taken ownership of the site. The Bank has received copies of the
non - compliance letters and will continue to work with the regulatory authorities to resolve these
issues.
The Pre - Construction Notification form, associated maps, and site plan are enclosed for
your review. The DWQ application fee has been submitted to your Raleigh office. Please contact

3. Project Description
3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application:
The project site is located within the Rutledge Subdivision and consists of an existing 45' wide paved road right -of -way
that is located between Phases 1 and 2 and a small dirt road in Phase 3. The paved road between Phases 1 and 2 was
constructed in -- 2008 and replaced a 25' wide dirt road (width based on aerial photos). This widening was constructed
without ACOE /DWQ authorization. The dirt road in Phase 3 has been in place for many years and acts as an earthen
dam. Phase 1 of the subdivision is largely developed. One home has been built in Phase 2 and no homes exist in Phase
3. Adjacent land use is undeveloped or residential.

3d. Explain the purpose of the proposed project:
The purpose of the project is to 1) obtain after - the -fact authorization for a road widening between Phases 1 and 2 and
make minor modifications to this crossing to prevent scouring and reduce erosion; and
2) repair the dirt road /earthen dam in Phase 3 in order to prevent the road from washing out and from sediment from
flowing into downstream wetlands /stream.
3e. Describe the overall project in detail, including the type of equipment to be used:
The applicant will make minor modifications to the paved road that is located between Phases 1 and 2 in order to stabilize
it. The existing shoulder will be regraded to a 2:1 slope. 25 CY of stone riprap will be added at the pipe outfall to prevent
scouring at the end of the existing culvert. Approximately 4 CY of riprap will also be added at the end of the brick walls.
Please see site plan for additional details.
At the earthen dam in Phase 3, if it is determined that the existing culvert is significantly damaged, the applicant will install
a coffer cell or sandbag system to temporarily dewater the area. The culvert will be removed and a new 18" RCP and
flashboard riser will be installed. Rip rap will also be installed. The coffers stem will then be removed.

1 a. Specifically describe measures taken to avoid or minimize the proposed impacts in designing project.
The paved road between Phases 1 and 2 was constructed where an old dirt road existed to reduce impacts to wetlands
and the stream. Also, the road was constructed with brick walls instead of side slopes to minimize impacts. For the
earthen dam in Phase 3, the proposed repair will occur mostly within the boundaries of the existing road. Therefore, very
little impact is needed.
1 b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques.
For the earthen dam in Phase 3, silt fencing will be installed to prevent erosion into adjacent wetland areas. A check dam
or similar structure will be installed downstream of the crossing prior to construction and will be removed once
construction is complete.
