Permitted MS4s are required to submit an annual report by October 31 for the previous fiscal
year (July 1 to June 30). If you are receiving this email, then DEQ does not currently have a
2020 Annual Report on file for your MS4 permit. Please provide the required annual report
within thirty (30) days to avoid enforcement action.
MS4 permit files are available on the Stormwater Program Laserfiche Repository.
If your MS4 was audited during fiscal year 2019-2020, then you can submit a status letter to
serve as the required annual report. An example letter is attached for your convenience.
Otherwise, the Stormwater Management Program Assessment (SWMPA) is the preferred
reporting option. Please see these links for support:

MS4 communities are also allowed to submit a narrative report that meets the requirements of
Part III, Part IV.B and Part IV.G of their permits.
