Submit a written (hard copy) cover letter asking to officially withdraw the DA Standard
permit (IP) previously submitted. In the same letter, indicate that NCDOT /Div 2 would like to
apply for a Cat Ex NW 23 for the Craven 176 bridge replacement. Please sign and date the
cover letter.
Include a statement that says a revised PCN is attached indicating application for the NW 23,
ensure that the PCN is complete (ie signed, dated identically with the letter, and all the
check boxes, impact blocks etc are correct); and reference all of the previously submitted
documentation as supporting materials for the proposed project.
When I receive a complete application and everything checks out, I will process it as soon as
possible; and we'll move on. I'll also talk with C. Rivenbark this am to ensure we are all on
the same page. Be advised that you are still responsible for coordinating and obtaining the
necessary authorizations from the other state, local and municipal agencies.

Tom, The PCE was not based on the NRTR report you mentioned but my current review of the NRTR
and discussion with Chris Rivenbark confirm that both documents are in agreement with each
other.

on the information contained in the May 2010 version of the NRTR authored by Tyler Stanton
and John S. Merrit of the NCDOT /NEU?
