Enclosed please find a copy of the Compliance Evaluation Inspection Report from the inspection conducted on July 12,
2011 by Hughie White of the Fayetteville Regional Office. Mr. Bill Stafford assisted with this inspection and his
assistance was greatly appreciated. The facility was found to be in Compliance with permit NC0023353.

ORC visitation records appeared to be complete and current. Calibration records appeared to be complete.
and properly documented. Laboratory data was reviewed and all data appeared to be correct as reported
on the DMR's. The grit removal system is still out of service. The mechanical bar screen was in good
condition. There was one aerator out of service that has been pulled from the lagoon and is currently -being
repaired. There was a minor amount of duckweed in the lagoon but there was some significant amount of
alligator weed floating in the lagoon. Be sure this does not get out of hand and possibly cause compliance
issues with the lagoon.

Comment: The flow meter was damaged by lightning several weeks ago and the
