On April 25, 2001, Mr. David Caldwell of the Mecklenburg County Department of Environmental
Protection (MCDEP) conducted an inspection of the subject collection system. This inspection was
conducted as part of a cooperative working agreement between Mecklenburg County and the Division of
Water Quality. The attached inspection report should be self-explanatory. The inspection revealed that the
facilities are being properly operated and maintained.
Also, attached is an excerpt from the regulation applicable to collection system permitting and a
copy of the relevant permit shell. Please review these two documents closely. Presently the subject system
is deemed permitted pursuant to 15A NCAC 2H.0227(d); however, in accordance with 15A NCAC
2H.0227(e), if inadequate operation and maintenance (O&M) of the system is documented or there is a
failure to implement an adequate O&M management program, then the Division may issue an individual
permit.

On May 8, 2003, Mr. Jeff Price of the Mecklenburg County Water Quality Program
(MCWQP) conducted an inspection of the subject collection system. This inspection was
conducted as part of a cooperative working agreement between Mecklenburg County and the
Division of Water Quality. The enclosed report should be self-explanatory.
This report is being issued as a Notice of Violation (NOV) because the every day
visitation of the pump stations is not being met in accordance to 15A NCAC 2H .0227 (d)(4), and
pump station # 5 does not utilize duplex pumps as required by Permit No.WQ0014177. Pursuant
to North Carolina General Statute (NCGS) 143-215.6A, a civil penalty of not more than twenty-
five thousand dollars ($25,000.00) may be assessed against a person who violates any
management practice established pursuant to NCGS 143-215. Should this Office make an
enforcement recommendation relative to the violations, you will be advised in writing.
It is requested that a written response be submitted to this Office by December 5, 2003,
addressing the violations noted above. In responding, please address your comments to Ms. Sonja
Williams. In addition, please send a copy of your response to Mr. Rusty Ro7Jelle, Mecklenburg

Mr. Dennis King (King Plumbing Repair Co.) maintains the eight (8) pump stations specified in
Permit WQ0014177. The gravity lines comprising the balance of the collection system are
maintained by Charlotte -Mecklenburg Utilities, and therefore were not evaluated during the
inspection. The pump stations need some maintenance, as detailed in the Pump Station/Siphons
Section. One of the pump stations is operated in a simplex configuration, although the individual
permit specifies that all stations be operated in duplex. This is a violation of Permit WQ0014177.
The pump stations are inspected daily (5 days per week). However, since the pump stations do
not have telemetry they are required to be inspected every day (365 days per year). This is a
violation of 15 A NCAC 2H.0227.

audible and visible high water alarms. The station control panels are locked for security. The
operator utilizes a portable generator for loss of power situations.
One (1) pump was pulled from the wet well for maintenance service. The station was
operating on one (1) pump only during the inspection.
The station's audible alarm operated properly during the inspection; however the visible
alarm failed to indicate a simulated high water condition during the inspection, reportedly due to a
failed bulb. Repair service to correct the non-functioning alarm is required immediately.
An emergency notification sign identifying a twenty-four (24) hour contact phone number
was posted and visible, as recommended in the previous inspection; however, pump -hour meters,
as recommended in the previous inspection, were still absent from the station. It is recommended
that pump -hour meters be installed to monitor pump function and efficiency as part of a
comprehensive preventive maintenance program.
It noted that the pump station's electrical wiring harness was damaged within the wet
well. This situation presents a corrosion hazard to the pump's wiring, and increases the likelihood
of pump failure and subsequent station overflow. The wiring harness should be replaced.
