Time: 7:59 AM, April 20, 2015 leachate line capped off. Time: 9:00 AM, April 20, 2015 reconstruction of service road began. Time: 8:50 AM, April 21, 2015 the pipe replacement began. Time: 7:30 PM, April 21, 2015 Leachate line was repaired. Time: 9:00 AM, April 22, 2015 the leachate line was re -pressured for test, no leaks were detected. NA DEPARTMENT OF ENVIRONMENT AND N7Tu.:'.FM;E)„URCES +:``'t'., "? fit'.,:'' _irJR':s��'F;=J NAY R,0 2000 Subject: Permit No. WQ0017771 Animal Shelter and Leachate Pump Station Cleveland County Health Department Wastewater Collection System Cleveland County In accordance with your application received December 9, 1999, and additional information received January 10, 2000, March 13, 2000, April 4, 2000, April 18, 2000, April 24, 2000, May 2, 2000, and May 5, 2000, we are forwarding herewith Permit No. WQ0017771 dated May 9, 2000, to the Cleveland County Health Department for the construction and operation of the subject wastewater collection extension. This permit shall be effective from the date of issuance until rescinded and shall be subject to the conditions and limitations as specified therein. Please note that if any other sewer connections other then the one contained within this permit are made to the two-inch line servicing the Mabry property, then the line becomes a pressure sewer and must be permitted by. the Division. This includes single building "taps" or any other pressure line connections. When an additional tie is made to this two-inch line is needed please submit PSSA 10/99 or later pressure sewer application as a modification to this permit. Please pay special attention to Condition 15 in which the Division is requiring a clean -out be placed at the end of two-inch line servicing the Mabry property, since the two feet per second velocity requirement is not met by the submitted design. Condition 16 requires both the leachate and the animal shelter pump station be equipped with the following safety devices: audible and visual high water alarms, and telemetry for each pump station. The Permittee and the Design Engineer is reminded of Permit Condition 11.e. which requires three feet of cover for all force mains and sewers permitted herein. Permitting of this project does not constitute an acceptance of any part of the plans and specifications that do not meet 15A NCAC 2H .0200 and the Division of Water Quality's Gravity Sewer Minimum Design Criteria adopted February 12, 1996 unless specifically mentioned herein. Division approval is based on the acceptance of the certification provided by a N.C. Professional Engineer on Page 6 -of the subject permit application form. It shall be the permittee's responsibility to ensure the plans and specifications meet the appropriate design criteria and rules. Failure to comply may result in penalties in accordance with North Carolina General Statute 143-215.6A — 143-215.6C, construction of additional facilities, and/or referral of the N.C. Professional Engineer to the licensing board. 1617 Mail Service Center, Raleigh, North Carolina 27699-1617 Telephone (919) 733-5083 Fax (919) 715-6048 An Equal Opportunity Affirmative Action Employer 50% recycled/10% post -consumer paper NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF ENVIRONMENT AND NATURAL RESOURCES RALEIGH WASTEWATER COLLECTION PERMIT In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO CLEVELAND COUNTY HEALTH DEPARTMENT Cleveland County FOR THE construction and operation of approximately 284 linear feet of eight -inch gravity sewer, a 12.8-GPM pump station with duplex pumps which must operate simultaneously, on -site audible and visual high water alarms external to any structure, an autodialer, a portable generator, a manual transfer switch and approximately 1,300 linear feet of two-inch force main to serve the Animal Shelter; a 45-GPM pump station with a single pump, on -site audible and visual high water alarms external to any structure, an autodialer, and approximately 7,600 linear feet of three-inch force main to serve the Leachate Pump Station; a 12-GPM pump station with a simplex pump, on -site audible and visual high water alarms external to any structure, 24 hours of wastewater storage, and approximately1,450 linear feet of two-inch force main with clean outs and , and approximately 1000 linear feet of 1.5-inch force main with water tight ferrous encasement pipe for all pipe without a bury depth of 36-inches that is not ferrous itself to serve a single residence on property of Franklin Mabry; the discharge of 18,910 GPD of collected domestic and leachate wastewater into the City of Shelby's existing sewerage system, pursuant to the application received December 9, 1999, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Environment and Natural Resources and considered a part of this permit. This permit shall be 'effective from the date of issuance until rescinded and shall be subject to the following specified conditions and limitations: 1. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, 15A NCAC 2H .0200, the Division of Water Quality's Gravity Sewer Minimum Design Criteria adopted February 12, 1996, the approved plans and specifications, and other supporting data unless specifically mentioned herein. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. The facilities must be properly maintained and operated at all times. 1
