Facility and Address: West Rowan High School Rowan -Salisbury Board of Education Post Office Box 2349 Salisbury, N.C. 28145 2. Date of Investigation: December 7, 1993 3. Directions to Site: West Rowan High School is on the left (west) side of Hwy. 801 2=0.5 mile north of the jct. of Hwy. 801 and SR 1526 (Sherrills Ford Rd.) in western Rowan County. 6. Discharge Point(s), List for all discharge Points: Latitude: 35° 42' 26" Longitude: 80° 38' 32" Attach a USGS Map Extract and indicate treatment plant site and discharge point on map. USGS Quad No.: E 16 NW 7. Site size and expansion area consistent with application: Yes. Adequate area exists for expansion, if necessary. 8. Topography (relationship to flood plain included): Gently rolling, 2-5%. The WWTP site does not appear to be located in or near a flood plain. 9. Location of Nearest Dwelling: Approx. 500+ feet from the WWTP site. Page Two 10. Receiving Stream or Affected Surface Waters: Withrow Creek a. classification: C b. Describe receiving stream features and pertinent downstream uses: Excellent flow observed in the receiving stream (6-8 feet wide x 6-10 inches deep) at the time of the site inspection. There are no other known dischargers and/or water intakes located downstream of this facility for a distance of at least 1.0 mile. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1 a. Volume of Wastewater: 0.010 MGD (Design Capacity) b. What is the current permitted capacity: 0.010 MGD c, Actual treatment capacity of current facility (current design capacity): 0.010 MGD d. Date(s) and construction activities allowed by previous ATCs issued in the previous two years: N/A e. Description of existing or substantially constructed WWT facilities: The existing WWT facilities consist of a septic tank/dosing tank with dual alternating siphons followed by dual surface sand filters with PVC manifolds. No disinfection is provided. f. Description of proposed WWT facilities: N/A g. Possible toxic impacts to surface waters: None expected. h. Pretreatment Program (POTWs only): Not NPeded. 2. Residual handling and utilization/disposal scheme: Septage is removed as needed from the septic tank by Myers Septic Service and disposed of by land application on fields permitted by the DEH. �. Other disposal options: N/A PART IV - EVALUATION AND RECOMMENDATIONS The permittee requests renewal of the subject Permit for the continued operation of the WWT facility serving the West Rowan High School. There have been no changes and/or modifications since the Permit was last reissued nor are any planned at this time.
