Division personnel have reviewed and approved your application for renewal of the subject permit. Accordingly,
we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the
U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
This final permit includes no changes from the draft permit sent to you on January 14, 2009.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this
letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to
obtain other permits which may be required by the Division of Water Quality or permits required by the Division
of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that
may be required.

This permit and the authorization to discharge shall expire at midnight on March 31, 2014.

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.

NOTIFICATION OF INTENT TO ISSUE A NPDES WASTEWATER PERMIT
On the basis of thorough staff review and application of NC General Statute
143.215.1 and 15A NCAC 02H.0109 and other lawful standards and regulations, the
North Carolina Environmental Management Commission proposes to issue a Nation-
al Pollutant Discharge Elimination System (NPDES) wastewater discharge perrnit to
the person(s) listed below effective 45 days from the publish date of this notice.
Written comments regarding the proposed permit will be accepted until 30 days after
the publish date of this notice. All comments received prior to that date are consid-
ered in the final determinations regarding the proposed permit. The Director of the
NC Division of Water Quality may decide to hold a public meeting for the proposed
permit, should the Division receive a significant degree of public interest.
Copies of the draft permit and other supporting information on file used to determine
conditions present in the draft permit are available upon request and payment of the
costs of reproduction. Mail comments and/or requests for information to the NC Divi-
sion of Water Quality at the above address or cat Dina Sprinkle (919) 807-6304 at
the Point Source Branch. Please include the NPDES permit number (below) in any
communication. Interested persons may also visit the Division of Water Quality at

Rowan -Salisbury Schools has applied for renewal of NPDES permit NC0034959 for
the West Rowan High School WWTP in Rowan County. This permitted facility dis-
charges treated wastewater to Withrow Creek in the Yadkin -Pee Dee River Basin.
Currently, no parameters are water quality limited. However, this discharge may still
affect future allocations in this portion of the Yadkin -Pee Dee River Basin.
Aqua North Carolina, Inc. has applied for renewal of NPDES permit NC0028941 for
the Pine Valley Subdivision WWTP in Rowan County. This permitted facility dis-
charges treated wastewater to the Selman Branch in the Yadkin Pee -Dee River
Basin. Currently BOD, ammonia nitrogen, and total residual chlorine are water quali-
ty limited. This discharge may affect future allocations in this portion of the Yadkin
Pee -Dee River Basin.
The Town of Cleveland has applied for renewal of NPDES perrnit NC0049867 for the
Cleveland WWTP in Rowan County. This permitted facility discharges treated
wastewater to Third Creek in the Yadkin -Pee Dee River Basin. Currently BOD, am-
monia nitrogen, fecal coliform and total residual chlorine are water quality limited.
This discharge may affect future allocations in this portion of the Yadkin -Pee Dee
River Basin.
RDH Tire and Retread Company (P.O. Box 187, Cleveland, NC 27013) has applied
for renewal of NPDES permit NC0075523 for its WWTP in Rowan County. This per-
mitted facility discharges contact cooling water and air compressor condensate to an
unnamed tributary to Beaverdam Creek in the Yadkin -Pee Dee River Basin. Cur-
rently oil and grease, total residual chlorine and total suspended solids are water
quality limited. This discharge may affect future allocations in this portion of the Yad-
kin -Pee Dee River Basin.

WWT facilities consist of a 0.02 MG septic tank, a 0.01 MG dosing tank, dual
surface sand filter beds, an effluent holding tank. Disinfection is not provided.
e. Description of proposed WWT facilities: There are no proposed WWT facilities at
this time.
f. Possible toxic impacts to surface waters: There are no toxic impacts expected.
2. Residual handling and utilization/disposal scheme: Residuals are removed as needed by a
local septage hauler.
3. Treatment plant classification: Class I (no change from previous rating).
4. Compliance Background: The most recent inspection conducted at this facility by MRO
staff did not document any permit or effluent violations.
PART III - OTHER PERTINENT INFORMATION
1. Special monitoring or limitations (including toxicity) requests: None at this time.
2. Important SOC or Compliance Schedule dates: This facility is neither under an SOC nor
is one being considered at this time.

The Rowan -Salisbury Schools Maintenance Department requests renewal of the subject
permit. There have been no (significant) changes to the permit and/or the existing WWTP since
the permit was last renewed nor are any changes proposed at this time.
Pending a finaleview by the Western NPDES Unit, renewal of the permit is
recommended.

In accordance with NPDES permit regulations we are making application for renewal of NPDES
Permit N0034959.
As requested, we are documenting any changes to the facility. These were made in 2004 under
permit WQ0023724. Please see letter of issuance attached. We made note of these due to the
fact that they occurred during the last permit renewal phase and wish to be positive they.are duly
recorded. In essence, the changes were upgrade and relocation of septic tank, dosing tank, and
lines to the filter bed. Please see attached drawings, C6.00.

The above permitted facility generates enough solids to have pumped and hauled by local
pump/haulbusinesses. Our grease interceptor is pumped once a year in June/July after
school recesses for summer. Our septic tank is generally pumped once in the spring and
once in the late fall. We also closely monitor to insure any additional pumping needs will
be immediately addressed.
