1 The DWQ s previous Request for More Information Letter stated You provided the Level
Spreader Supplement Form for each proposed level spreader but failed to fill out Part III
Required Items Checklist Please fill out the Required Items checklist and be certain to provide
ALL items at the requested scales If you fail to provide these items correctly in your next
submittal the project will be returned
The submittal received by the DWQ on July 20 2012 did not include completed Required Items
checklists nor did it include all of the required items at the correct scales Therefore this project
is returned
