I. INTRODUCTION:

This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H
.0800 for the analysis of environmental samples.
II. GENERAL COMMENTS:
This is .a new laboratory. The lab has developed bench sheets, logs, and forms. All equipment
and reagents needed to analyze the samples, .for which certification is being sought, has been
obtained. Performance evaluation samples have been ordered. A Quality assurance
document, including method standard operating procedures, has been prepared.
III. DEFICIENCIES, REQUIREMENTS, RECOMMENDATIONS, and COMMENTS:
No deficiencies were noted.
IV. PAPER TRAIL:
There was no data to review at this time.
V. CONCLUSION:
The laboratory staff has done a very good job of preparing the lab for certification. Upon
receiving acceptable results on the performance evaluation samples and payment of the
appropriate fees, it is recommended that certification should be issued. Approximately three
months after obtaining certification another inspection will be conducted

procedures has been reviewed. The quality control measures taken in reference to the comments -
cited in the January 2, 2004 inspection report are acceptable. This report does not attempt to
comprehensively address all certification requirements prescribed in the North Carolina
Administrative Code (15A NCAC 2H .0800), promulgated methods, and applicable Code of
Federal Regulations (40 CFR Part 136). It is the responsibility of the certified laboratory to address
any required changes in Wastewater/Groundwater Laboratory Certification application information,
and ,to correct the Laboratory Standard Operating Procedures and the Laboratory Quality
Assurance Manual when requirements of the above cited codes are. not in -compliance.
If a future inspection should. reveal that the deficiencies/requirements cited in the inspection
report were not corrected, enforcement actions will be recommended in accordance with 15A
NCAC 2H .'0800.

We have made immediate changes in order to correct the following.

A laboratory control standard is now being utilized with each daily sample. The

III d Fecal Coliform
Since the date of the inspection, comparison testing for new membrane filters has

With a colony count between 20 to 60 colonies. No other comparison tests have
Been needed, but the same procedure for filters and media will be followed as
The lab has received required new lots.
All of the above was immediately changed in order to comply with all regulations and I

This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for
the analysis of environmental samples.
ll. GENERAL COMMENTS:
The staff is congratulated for doing a good job of maintaining the laboratory program. The facility
analyzes samples for the Corpening Creek WWTP, the Catawba River WWTP, and the City's WTP, The
laboratory is spacious and well equipped. All equipment is well maintained. Records are well kept and
most data appeared accurate. Some further quality control procedures need to be implemented.
III. DEFICIENCIES, REQUIREMENTS. COMMENTS AND RECOMMENDATIONS:
Total Residual Chlorine -Catawba River WWTP and WTP
NOTE: The WTP has just been issued a new permit requiring low level chlorine analyses. It is
anticipated that both WWTPs will be receiving new permits in the next few months that will also require
low level testing. New permits provide for an 18 month period to come into compliance.
COMMENT: The facilities do not have meters capable of reading to low concentrations. At the present
time pocket colorimeters are used. The laboratory has begun the process of budgeting for and
obtaining the items needed. The Corpening Creek facility may also need some equipment upgrades to
perform the low level analyses.
REQUIREMENT: Each facility must have glassware, chemicals, supplies, equipment, and a source of
distilled or deionized water that will meet the minimum criteria of the approved methodologies. Ref: 15A
NCAC 2H ,0805 (g) (4).
REQUIREMENT: In accordance with the directive from this division, a hand held pocket colorimeter
will be allowed to have a minimum reporting limit of 100 ug/I. For any facilities with a limit less than this,
the analysis must be performed with a spectrophotometer. Ref: Division of Water Quality letter dated
August 14, 2001.

Ammonia Nitrogen
A. COMMENT. A few instances of the concentration of the blank being very near and one instance of
the blank being greater than the concentration of the low standard were noted. The signal reading
(absorbance value) for the low standard is not sufficiently greater than the blank. This makes the
Practical Quantitation Limit (PQL) or reporting limit questionable due to unviable signal readings.
REQUIREMENT: North Carolina policy requires the concentration of the blank must not exceed
approximately 50% of the PQL. The lab must obtain a signal in which the constituent concentration in
reagent water is 2(1.645) s above the mean of blank analyses (Lower Limit of Detection). The
constituent concentration that produces a signal sufficiently greater than the blank, that it can be
detected within specified limits, by good laboratories, during routine operating conditions, is the Limit of
Quantitation. Typically it is the concentration that produces a signal 10s above the reagent water blank
signal. In most cases, this requires an absorbance value greater than ,005 units, preferably greater
than .010 units. Ref: Standard Methods, 18th Edition - Method 1010 C. And 1030 E.
Suspended Residue
B. COMMENT. Occasionally, sufficient sample volume is not analyzed to obtain the method required
weight gain. A maximum volume of 500 mi is generally used. A few instances of a smaller volume than
500 mi were noted where the weight gain was less than the required 1.0 mg, and the sample was
reported as < 2mg/L. The value must be reported as less than the appropriate value based upon the
volume used for that particular sample not based upon the maximum volume ever used of 500 mi.
REQUIREMENT: When analyzing suspended residue, sufficient sample must be filtered to yield at
least 1.0 mg of residue. Ref: EPA-600/4-79-020 Methods Manual, March 1983 - Method 160.2-2.IV.
IV. PAPER TRAIL INVESTIGATION:
No paper trail was performed.
V. CONCLUSIONS:
Correcting the above cited comments will help this lab to produce quality data and meet certification

On December 12, 17, 2004 Gary Francies performed our maintenance inspection on
our lab Listed below in order is our reply on how we have taken corrected actions.
TOTAL .RESIDUAL CHLORINE
We have submitted to the town an increase to the budget to purchase the equipment
needed to correctly analize the new CL2 Iow level limits. We have contacted several
vendor's to insure we are getting the equipment that is needed to assure accuracy of the
analysis
AMMONIA NITROGEN
Upon experiencing questionable blank concentrations 3 of 14 times the analysis has
been run. We are checking the quality of the deionized water. In two of the three cases DI
cartridges were changed shortly after the test dates.. We are consulting with Barnstead
Tech Support , they suggested that we change the ultrapure cartridge 2 times to 1 of the
organic cartridges. Additionally Barnstead will run analysis on our water to determine if
additional cartridges are needed to maintain the quality of our DI water. We will keep a
constant check we have already purchased the cartridges needed and will change them in
the order.Barnstead has recommended. In the event of a blank exceeding the
approximately50% of the PQL , the blank will be re -run as a corrective measure. The
other possible corrective action would be to increase the concentration of the low

standard, if the above steps do not continue to correct the problem. We have not
experienced any problems on our analysis during the month of January.
SUSPENDED RESIDUE
A larger volume of sample is now being collected by our lab this should insure that the
required yield of at least 1.0mg of residue is reached. As a corrective action the test will
be rerun if at all possible, in the event that the yield is less than 1.Omg of residue. The.
reported values shall be based upon the volume used, not the previous used maximum
volume of the 500mils.

This laboratory was inspected to verify its compliance with the requirements of 15A NCAC 2H .0800 for
the analysis of environmental samples.
II. GENERAL COMMENTS:
The staff is congratulated for doing a good job of maintaining the laboratory program. The facility
analyzes samples from the Corpening Creek WWTP, Catawba WWTP, and the city's WTP. The
laboratory is of adequate size and well equipped. All equipment is well maintained. Records are .well
kept and most data appeared accurate. Some further quality control procedures need to be
implemented.
III. DEFICIENCIES, REQUIREMENTS, COMMENTS, AND RECOMMENDATIONS:
Catawba WWTP — Field parameters
A. COMMENT: The time of sample collection/analysis is not clearly documented.
REQUIREMENT: Data pertinent to each analysis must be maintained for five years. Certified data must
consist of date collected, time collected, samples site, sample collector, and sample analysis time. The
field benchsheets must provide a space for the signature of the analyst, and proper units of measure
for all analyses. Ref: 15A NCAC 2H .0805 (g) (1).
Ammonia nitrogen
Note: Since the last inspection the laboratory has switched from the spectrophotometer to the ion
electrode method.
B. COMMENT: A laboratory control standard (LCS) is not analyzed.
REQUIREMENT: Analyze one known standard in addition to .calibration standards each day samples
are analyzed to document accuracy. This laboratory control standard must be from a second source. It
is recommended that this laboratory control standard, like the calibration verification standard, has a
mid -range concentration and not vary by more than ± 10%. If it does the analysis is out of control. The
analysis must be terminated and the problem corrected. Ref: 15A NCAC 2H .0805 (a) (7) (B), (F), and
Standard Methods, 18th Edition - Method 1020 B.3
C. COMMENT: The calibration verification standard (CVS) is not of a mid -range concentration.

NOTE: Currently the meter is calibrated over a range of 0.1 to 10 mg/I and the CVS has a
concentration of 1.0 mg/I.
REQUIREMENT: For analyses that require a series of standards, a mid -range standard must be
analyzed with each group of samples to verify the standard curve. When the mid -range standard varies
by more than ± 10%, or is outside the established control limits, whichever is lower, the analysis is out
of control. The analysis must be terminated and the problem corrected. Ref: 15A NCAC 2H .0805 (a)
(7) (F) and (1).
RECOMMENDATION: Since the concentrations of the majority of the samples is low and rarely
exceeds 5.0 mg/l, it is recommended that the meter be calibrated with standards from 0.1 to 5.0 mg/l.
The concentration of the CVS would then be approximately 2.5 mg/I.
Fecal coliform
D. COMMENT: No comparison test is conducted before a new lot of consumables are put into use.
NOTE: Comparison testing is performed when new filters and pads are obtained but not new media.
REQUIREMENT: It is required that when a new lot of culture medium, pads, or membrane filters is to
be used, a comparison of the current lot in use, (reference lot) against the new lot (test lot), be made.
As. a minimum, make single analyses on five positive samples. Ref: Standard Methods, 18th Edition -
Method 9020 B.3d. (page 9-7)
RECOMMENDATION: The concentrations of the majority. of the samples used for the comparison test
are low and usually 20-60 colonies are not obtained. It is recommended that a culture positive sample
that a dilution yielding 20-60 colonies be used for the comparison test.
IV. PAPER TRAIL INVESTIGATION:
A review of data was conducted. This consisted of comparing laboratory bench sheets and contract lab
reports to Discharge Monitoring Reports (DMRs) submitted to this Division. Data were reviewed for the
Corpening Creek WWTP, Catawba WWTP, and the city's WTP for September 2003. No errors were
noted. It appears the facility is doing a good job of accurately transcribing data.
V. CONCLUSIONS:
Correcting the above cited comments and implementing the recommendations will help this lab to
produce quality data and meet certification requirements. Please respond to all lettered comments.

Your letters, received August 14 and 20, 2002, concerning corrective actions for your analytical procedures
has been reviewed. The quality control measures taken in reference to the comments cited in the August 1, 2002
inspection report are acceptable with the following clarification. Your response to Comment E., concerning the use of
the incorrect reagent at the WTP for the measurement of Total Residual Chlorine, not Free Chlorine, was not
completely clear. I am assuming that the DPD for Total Residual Chlorine is now used. Also, We have received and
reviewed the information you provided for the ammonia nitrogen comparative results performed in your laboratory.
The results are acceptable for the requirements of your laboratory certification.
This report does not attempt to comprehensively address all certification requirements prescribed in the North
Carolina Administrative Code (15A NCAC 2H .0800), promulgated methods, and applicable Code of Federal
Regulations (40 CFR Part 136). It is the responsibility of the certified laboratory to address any required changes in
Wastewater/Groundwater Laboratory Certification application information, and to correct the Laboratory Standard
Operating Procedures and the Laboratory Quality Assurance Manual when requirements of the above cited codes are
not in compliance.
If a future inspection should reveal that the deficiencies/requirements cited in the inspection report were not
corrected, enforcement actions will be recommended in accordance with 15A NCAC 2H .0800.

II. GENERAL COMMENTS:
The laboratory is spacious and well equipped. All equipment is well maintained. Records are well kept
and most data appeared accurate. The staff has done a very good job of implementing the laboratory
program. The laboratory is analyzing samples for the Corpening Creek WWTP, the Catawba River
WWTP, and the city's Water Treatment Plant. On site testing at the latter two facilities is also being
performed under the laboratory's certification. Some further quality control procedures need to be
implemented.
III. _ DEFICIENCIES, REQUIREMENTS, RECOMMENDATIONS, and COMMENTS:
Quality Control -Duplicates
COMMENT: Control limits for duplicate analyses, based upon the laboratory's data, have not been
established yet. The lab is just now compiling enough data points to be able to do this. The staff was
beginning to work with the data to establish limits. This needs to be completed.
REQUIREMENT: Control limits for duplicate sample analyses must be established. No firm acceptance
values have been established, documented, or evaluated. against results obtained on analyses. The
results obtained should. be documented on the bench sheet and evaluated against the acceptance
criteria. Any samples not meeting the criteria must be reanalyzed if possible. If this is not possible, the
data must be flagged on the laboratory reports and Discharge Monitoring Reports (DMR) as all quality
control requirements not met.
REQUIREMENT: Each laboratory shall develop and maintain a document outlining the analytical quality
control practices used for the parameters included in their certification. Supporting records shall be
maintained as evidence that these practices are being effectively carried out. Ref: 15A NCAC 2H .0805

Fecal Coliform
A. COMMENT: Insufficient comparison testing is conducted before a new lot of consumables are
put into use.
REQUIREMENT: It is required that when a new lot of culture medium, pads, or membrane filters is to
be used, a comparison of the current lot in use, (reference lot) against the new lot (test lot), be made.
As a minimum, make single analyses on five positive samples. Ref: Standard Methods, 18th Edition -
Method 9020 B.3d.(page 9-7)
COMMENTS: One positive culture, for each material, was analyzed when a new lot number of media
and filters were put into use. No comparison testing was performed when a new lot number of media
pads were put into use.
Suspended Residue
B. COMMENT: Sufficient sample volume is not always analyzed to obtain the method required
weight gain. A maximum volume of 200 ml is generally used. The lab's application on file indicates a
reporting limit of 1.0 mg/I. This reporting limit must be changed to 5.0 mg/I if a maximum sample
volume of 200 mis is to be continued. If a larger sample volume is to be filtered, the application needs
to be updated to reflect the appropriate reporting limit. In instances where the weight gain is less than
the required 1.0 mg, the value must be reported as less than the appropriate value based upon the
volume used.
REQUIREMENT: When analyzing suspended residue, sufficient sample must be filtered to yield at
least 1.0 mg of residue. Ref: EPA-600/4-79-020 Methods Manual, March 1983 - Method 160.2-2.IV.
Ammonia Nitrogen
C. COMMENT: The lab has not completed the ammonia distillation comparison study.
REQUIREMENT: Laboratories shall analyze initially four samples distilled and undistilled from each
discharge and, thereafter, analyze two samples a year from each effluent. Additional .samples must be
analyzed comparatively if the above samples results do not indicate approximately the same values for
distilled and undistilled samples. Ref: Federal Register, July 1, 1995 40 CFR 136; Ammonia Footnote
6.
COMMENTS: The lab sent the required four samples to its contract lab for the comparison study, but
only received back the results from three samples. Those results were acceptable. The lab tried to get
the fourth set but has been unsuccessful so far. If the data can not be obtained another sample must
be done to complete the comparison study. Please submit the results of the fourth sample to this office
for review.
Catawba River WWTP -Total Residual Chlorine
D. COMMENT: A mid -range calibration verification standard is not always analyzed.
REQUIREMENT: For analyses that require a series of standards, a mid -range standard must be
analyzed with each group of samples to verify the standard curve. When the mid -range standard varies
by more than t 10%, or is outside the established control limits, whichever is lower, the analysis is out
of control. The analysis must be terminated and the problem corrected. Ref: 15A NCAC 2H .0805 (a)

COMMENT: The lab has been analyzing a calibration verification standard for the field meter once per
week, not each day samples are analyzed as required.

E. COMMENT: Free Chlorine "is being measured not Total Residual Chlorine as required by the
permit.
REQUIREMENT: The proper reagents must be used in,the analysis. Ref: Federal Register, 40 CFR-
136.3, July 1, 1995.
F. COMMENT: Sample holding times are not documented. Currently the time samples are
collected is documented but not the time of analysis. Both must be documented to show that the
holding time has been met.
REQUIREMENT: All samples must be analyzed within the EPA approved holding time of fifteen
minutes. Ref: Federal Register, 40 CFR-136.3, July 1, 1995.
REQUIREMENT: Supporting records shall be maintained as evidence that these practices are being
effectively carried out. All analytical records must be available for a period of five years. Ref: 15A
NCAC 2H .0805 (a) (7) and (a) (7) (G).
G. COMMENT: The photometer for residual chlorine has not had the internal calibration curve
verified across the range of the meter with five standards.
REQUIREMENT: A verification of the internal calibration curve must be performed annually. This is
performed by verifying the readings of five standards. The concentrations of the five standards should
bracket the concentration of the samples analyzed. One of the standards should have a concentration
equal to or below the lower reporting concentration. It is recommended that the values obtained not
vary by more than 10% of the known value for standards greater than 50µg/L and 25% of the true
value for standards less than 50µg/L. If the stored program readings vary by more than the above
acceptance criteria, the stored calibration program should not be used for quantitation until
troubleshooting is carried out to determine and correct the source of error. Ref: Standard Methods, 18th
Edition - Method 4500-CI G., Method 1020E Quality Control (5) page 1-5and Method 1070D 2.
H. COMMENT: A mid -range calibration verification standard is not analyzed.
REQUIREMENT: See requirement for Comment D above.

REQUIREMENT: For routine work, use a pH meter accurate and reproducible to 0.1 pH units with a
range of 0 to 14 and equipped with a temperature compensation adjustment. Analyze a third buffer
below pH 10, approximately three pH units different from the second. The reading should be within 0.1

units of true value. The results of the third buffer must be documented. Ref: Standard Methods, 18th
Edition - Method 4500 - H+ B (2) and (4) (a) and Ref: 15A NCAC 2H .0805 (a) (7).
COMMENTS: The permit no longer requires pH to be reported. The facility has been voluntarily
reporting these data. If they continue to do so the above two items -must be done.
IV. PAPER TRAIL:
A review of data was conducted. This consisted of comparing laboratory bench sheets and contract lab
reports to DMRs submitted to this Division. Data were reviewed for all three facilities for these months:
February and March 2002. No errors were noted. It appears the facility is doing a - good job of
accurately transcribing data.
V. CONCLUSION:
This laboratory is doing a good job overall. Correcting the above items should help to produce quality
