REQUEST FOR MORE INFORMATION
Dear Mr. Beacham:
On April 26, 2007 the Division of Water Quality (DWQ) received your application dated April 23, 2007 to impact
0.26 acres of wetlands to construct the proposed roadway. The DWQ has determined that your application
was incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional
information in order to process your application to impact protected wetlands and/or streams on the subject
property. Therefore, unless we receive the additional information requested below, we will have to move
toward denial of your application as required by 15A NCAC 2H .0506 and will place this project on hold as
incomplete until we receive this additional information. Please provide the following information so that we
may continue to review your project.
Additional Information Requested:
1. Application Information
Application signed by someone other that applicant. A signed and dated copy of the Agent
Authorization letter must be attached if the Agent has signatory authority for the owner/applicant.
2. Impact Justification (Avoidance and Minimization)
Specifically under Avoidance and Minimization placement of pile below streambed or wetland elevation
for aquatic passage during low flow conditions was not addressed. A cross sectional drawing of the
pipe placement should be included.
3. If wetlands are within a FEMA flood area pipe size and placement should accommodate flood
conditions.
4. Pipe size is inconsistent in application, Section VI Proposed Impacts, states the use of two 18 " CMP's
(Corrugated Metal Pipes) and Section XI Stormwater, states the use of two 36" CMP's.

5. Stormwater
Description states that the two pipes will be placed 120' apart. Plan drawing show pipes less than 10
feet apart. Description and plan drawings should be consistent of one another.
Please respond in writing within three weeks of the date of this letter by sending this information to Roberto
Scheller of the DWQ Washington Regional Office. If we do not hear from you within three weeks, we will
assume that you no longer want to pursue this project and we will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters or
protected buffers. Please be aware that any impacts requested within your application are not authorized (at
this time) by the DWQ. Please call Roberto Scheller at 252-948-3940 if you have any questions regarding or
would like to set up a meeting to discuss this matter.
