Dear Mr Nivens
On June 11 2012 the North Carolina Division of Water Quality (Division) received a copy of an application for a 401
General Water Quality Certification install a fill in streams at the Martin Marietta Materials Thomasville Quarry site in
Thomasville NC in Davidson County Additional information is required in order that we may determine your project s
compliance with 15A NCAC 2H 0500 The Division will require this information in order to process your application to
impact protected wetlands and/or streams on the subject property Therefore we will place this project on hold as
incomplete until we receive this additional information and if we do not receive the additional information requested
below we will have to move toward denial of your application as required by 15A NCAC 2H 0506 Please provide the
following information so that we may continue to review your project
1 We have discussed that a site visit is necessary in order to adequately review the impacts from this project You
have indicated that you will contact me in the near future to coordinate a site visit Please coordinate the site visit
within the next 30 days so that we may efficiently facility permit review
2 Please provide a more detailed scaled drawing for the area of the existing culvert under the road the proposed
culvert area and downstream to the culvert under the railroad This drawing should accurately reflect the
alignment of the stream between the existing culverts and the alignment of the proposed culvert
Per the requirements of GC 3890 this project must provide a Stormwater Management Plan for the additional
built upon area associated with the expanded entrance road This SMP must be designed to remove a minimum of
85 percent Total Suspended Solids from the stormwater generated by this project For each proposed BMP
please provide a completed BMP Supplement Form with all the required items The DWQ will allow you to treat
the stormwater from an equivalent amount of built upon area if that is more cost effective on your site

from you in 30 calendar days we will assume that you no longer want to pursue this project and we will consider the
project as returned
