North Carolina Department of Transportation (NCDOT) is submitting monitoring and reporting for
2020 as specified in the above referenced permit to be submitted electronically annually (Condition
IV.6). NCDOT compiled this report using data gathered during the compliance year by NCDOT
and our contractors. This data includes: I) Diamond Grinding Slurry (DGS) and Hydrodemolition
Operation Slurry (HOS) Analyses (Permit Condition IV.2); and II) DGS and HOS pH and Land
Application Logs (Permit Conditions IV.2 and IV.5).

6. When grinding the middle and outside lanes or areas that slurry cannot be land applied on
the shoulder due to drainage structures or water ways, the slurry will be vacuumed up and
hauled to an acceptable location to land apply on NCDOT right of way.
7. pH sampling will be done to ensure that the pH is below 12.5.1f pH is above 12.5 Inbriatic
acid will he added to achieve a level below 12.5 prior to hauling or land applying the slurry.
& pH will be tested with calibrated pH testing meter.
9. There will be at least one pH test taken in the field per day. The pH meter will be calibrated
at method and frequency determined by the manufacturer recommendation. pH meter will
be calibrated, and ply test done twice daily will be logged. Any slurry hauled off o the
roadway will have a pH test done per load.
10. A soil test will be done to determine appropriate agronomic Kates for soil application. This
will be done by a licensed soil scientist in multiple locations to get a good overall
representation of the soil characteristics of the project.
11. A percent solids test will be done on the slurry first shift to determine the amount of solids
being land applied.
12. The application rate of slurry will be based on agronomic needs.
13. Slurry application will stay 25 feet away from drop inlets, and sand bags will be applied to
these areas in the event a spill occurs within the 25 feet.
14. if a spill does occur the operation will shut down, the leaf will be corrected, and the
sandbags will be placed to contain the leak. The vacuum system on the machine will them
be utilized to recover the leaked slurry.
15. SIurry application will stay 100 feet away from any streams or creeks.
16, Areas where slurry will be applied on the projects will be marked prior to work in
agreeance with NCDOT.
17. Areas where slurry will application will be avoided will be marked prior to work with a
delineator, lathe, or spray paint in agreeance with. NCDOT.
18. SDO's or jurisdictional areas will be marked prior to work.
19. NCDEQ will be notified prior to diamond grinding and slim disposal operations.
20. DOS sampling will be done using EPA method 9040 with at least one filed representative
sample taken per day.
21. A log book will be used to enter the pH results daily, and then provided to NCDOT on a
weekly basis.
22. The Engineer/and or DEO will be notified immediately after a spill occurs.
23. We will adhere to the NCDOT Guidelines on the management and disposal of concrete
grinding residues.
24. The following personnel will be the qualified pH testers on the project: Kyle Miller, Cody
Dunham, James Johnson, Bryon Lee, Shane Lee, Nick Lattuca, and Carl Comer.
25. Will adhere to the requirements in NCDOT permit No. WQ003579. This is including
having the Q&M requirements.
26. Slurry shall not be applied if the soil is flooded or in a condition that runoff of the slurry
would occur. Slurry will also not be applied within 24-hours following a rainfall event of
0.5 inches or greater,
27. Materials that pass the paint filter test are deemed concrete solids and may be used as
beneficial fill.
28. A copy of the spill control plan will be maintained in all transport application land vehicles.
29, An equipment inspection log will be maintained. Equipment inspections should be
performed prior to each bulb residual distribution of land application. event.
30. We c,�mplete and maintain a Iand application log.
31. We will apply the slurry to not exceed the application rates that are attached to this plan, by
Andy Miller (National Resaumes Conservation Service, Davidson Co.).
32. Test on the slurry determined the percent solids to be 12%, pfi averaged 9.3%.
