The following information is provided: PPrmittPP- Terra Renewal Services, Inc. P.O. Box 399 Dardanelle, AR 72834 Preparer & Contact: Terra Renewal Services, Inc David C. Coyle; Environmental Manager P.O. Box 1746 Watkinsville, GA 30677 phone: 479-747-8192 email: david.coyleCcDterrarenewal.com These items are included with the report (in order): 1) The Annual Land Application Certification Form (ACF) 2) The Annual Residuals Sampling Summary Form (RSSF) and Annual Residuals Sampling Summary Form — B (RSSF-B) for each residuals source 3) The Annual Metals Field Loading Summary Form (MFLSF), Annual Land Application Field Summary Form (FSF), and Annual Land Application Field Summary Form Supplement (FSF- SUPP) 4) Lab Analyses for Residuals: — Nutrients, Metals, TCLP, Other — Fecal Density (Seven Set) — Vector Attraction Reduction Data (30 Day Bench) 5) Annual Pathogen and Vector Attraction Reduction Form for NCAC 2T Rules (PVRF 2T) 6) Soil Test Reports (for fields applied in 2020) Page 2 2020 Annual Report WQ0038835 Residuals were land applied and incorporated per the conditions of the permit. PAN application rates were established based on a spring corn crop in 2021. These fields were planted in small grains within five days of conclusion of the land application event (mid -December), intended as a cover crop for the purpose of site stability and to assimilate any nitrate mobilized during winter months. The cover crop will be "burned down" and incorporated prior to planting corn in spring of 2021. On the Annual Land Application Certification Form (ACF), the option "No" is selected under Part B — Annual Compliance Statement for the statement "Facility was compliant during calendar year 2020 with all conditions of the land application permit issued by the Division of Water Resources". The reasons for this are as follows: Item-2 — "Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at least 6.0 or the limit specified in the permit. " — Soil samples were collected at the time of the application event. Results for both fields applied showed soil pH < 6.0. This project concluded in early December 2020. Following application, the information regarding the need for pH adjustment was provided to the farmer. Field conditions were too wet in late December 2020 through February 2021 to allow for lime application. Lime application at the rates recommended by the Agronomist is planned for both fields prior to planting corn in spring 2021. Records of the application (lime tonnage tickets and dates) will be provided to the Division as an addendum to this report once complete. All other conditions listed under the Compliance Statement on Form ACF were met. If additional information is required or there are any questions regarding this annual report, please contact David Coyle via the information listed above. Thank you for your cooperation and attention to this matter. Sincerely, David C. Lot Number: VK07060 This Report of Analysis contains the analytical result(s) for the sample(s) listed on the Sample Summary following this Case Narrative. The sample receiving date is documented in the header information associated with each sample. All results listed in this report relate only to the samples that are contained within this report. Sample receipt, sample analysis, and data review have been performed in accordance with the most current approved The NELAC Institute (TNI) standards, the Pace Analytical Services, LLC ("Pace") Laboratory Quality Manual, standard operating procedures (SOPs), and Pace policies. Any exceptions to the TNI standards, the Laboratory Quality Manual, SOPs or policies are qualified on the results page or discussed below. Where applicable, all soil sample analysis are reported on a dry weight basis unless flagged with a "W' qualifier If you have any questions regarding this report please contact the Pace Project Manager listed on the cover page. The pH analysis associated with the sample(s) in this report was/were performed after the method recommended 15 minute holding time. Metals by ICP Due to low % solids the initial amount of these samples was 3 times the standard procedure amount per PM's request. The LOQ has been adjusted accordingly. % Volatile Solids The following sample was analyzed outside of analytical holding time due to the test having been added after the sample's expiration: VK07060-001. Paint Filter Samle -001 has been qualified with an "H", however, there is not a method defined hold time for this parameter. For best results, mix the lime into the top 6 to 8 inches of soil several months before planting. BS% % CEC occupied by basic cations For no -till or established plantings where this is not possible, apply no more than 1 to 1.5 ton/acre (50 Ib/1000 sq ft) at onE Ca% % CEC occupied by calcium time, even if the report recommends more. You can apply the rest in similar increments every six months until the full rate CEC cation exchange capacity is applied. If MG is recommended and lime is needed, use dolomitric lime. Cu-I copper index ESP exchangeable sodium percent Fertilizer HM% percent humic matter Recommendations for field crops or other large areas are listed separately for each nutrient to be added (in units of K-1 potassium index lb/acre unless otherwise specified). Recommendations for N (and sometimes for B) are based on research/field studies K20 potash for the crop being grown, not on soil test results. K-1 and P-1 values are based on test results and should be > 50. If they Mg% % CEC occupied by magnesium are not, follow the fertilizer recommendations given.
