This assessment is based upon the following facts: A review has been conducted of the self - monitoring data reported for April 2014. This review has shown the subject facility to be in violation of the discharge limitations found in NPDES Permit No. NCO047091 . The violations are surmnar. ized in Attachment A to this letter.
