Individual Permit request for the above referenced property
1 The Public Notice did not discuss whether other sites had been evaluated within the alternatives
analysis If this information was shared with the USACE this Office would like to comment
from the USACE regarding the results of this analysis
2 This Office requests that the applicant please re submit site plans on full plan sheets at a scale of
no smaller than 1 =50 with topographic contours shown
3 This Office requests that the applicant please locate all isolated or non isolated wetlands streams
and other waters of the State as overlays on the site plans
4 This Office requests that the applicant please provide cross section details showing the provisions
for aquatic life passage
5 This Ottice requests that the appiicant please indicate all -%Cdand impac*s incl-ding fill slopes on
the site plan
6 This Office requests that the applicant please provide a qualitative indirect and cumulative impact
analysis for the project
7 This Office notes that all streams listed in application are classified as C Trout or are tributary to
C Trout waters Stormwater control BMP s that addi ess the requ►remcnts of*ris attr iba*e shall
be implemented
8 This Office requests that that applicant discuss which if any of the culverts identified for
