Box 1776 Goldsboro, NC 27533 4,0 Fast Track Application 2021 Sewer System Improvements CN 3 — Kornegay Office PS & FM Fork Township Sanitary District Wayne County, NC c'vzoltet, A request has been made by David Kornegay to obtain some assurance that upon the occurrence of an on -site wastewater disposal failure at property he is interested in buying, he may be able to tie to the Town of Mount Olive's wastewater collection system in the future. This connection would be by installation of an on -site duplex grinder pump station and force main installed in NCDOT right-of-way to a manhole approximately 482 LF from the subject site. Per our conversations, a "Construction Only" application in the Town of Mount Olive's name is being submitted for zero (0) gallons of flow. It is understood at some point in the future that if his on -site disposal system fails, Mr. Kornegay may contact the Wayne County Health Department for their ruling on whether an on -site system could be used at that point. If on -site septic is not possible, the Town of Mount Olive could then apply to have the permit modified to a permit with some amount of flow and installation of the improvements could occur. The property is currently a vacant residential style structure that has most recently been used as an office space. The "flow" permit in the future would likely request 360 gpd which represents flow from a three (3) bedroom residence based on 2T rules.
