This review has shown the subJect facility to be in violation of the discharge limitations found in PDES Permit No. NCO074799. The violations are sure triarized in Attachment A to this letter. Based upon the above facts, I conclude as a matter of law that Pines -%,IlIP violated the tern -is, conditions., or requirements of NPDES Pern-fit No. NCO074799 and North Carolina General Statute (G,S.) 143-215. l(a)(6) in the manner and extent shown in Attachment A.
