application submitted for the above referenced property Additional information will be required in order to
complete the review process
Please provide the following information so that we may continue to review your project

1 Application item number 5 indicates purpose of the proposed work as shoreline
stabilization and sea wall protection There are no attachments showing the needed
stabilization This agency has no way of determining the need for stabilization from the
submitted application You must submit attachments, such as drawings, photographs
(with scale), etc for review as part of your application
2 Please provide a valid authorization form granting agent permission to sign the application
form by the applicant (Attached)
3 Please explain and provide a brief narrative for the following items How do you plan to
work in the dry and how do you plan to minimize and avoid any surface water impacts
during this protect, such as turbidity, sediment to the Lake, etc Please provide a timeline
that includes a start date for your protect, timing of the Lake Level withdrawal if
applicable, and a construction sequence Please provide a site map with the location of the
impacted area and additionally address diffuse flow of storm water issues with this lot
