This assessment is based upon the following facts, A review has been conducted ofthe self - monitoring data reported for January 2012. This review has shown the sui ject facility, to be in violation of the discharge limitations mund in NPDES Permit No. NCO020401 x The violations are summarized in Attachment A to this letter. Based upon the above facts, I conclude as a matter of law that the City of Hickory violated the terms, conditions, or requirements of NPDES Permit No. NCO020401 and North Carolina General.
