Our team has done some research and we have determined that you owe the $100 annual permit fee for
Certificate of Coverage NCC200338.
The COC was issued on 1/31/2020, and you were billed on 1/4/2021. The fee was due 2/4/2021.
This allowed the Town the month of January to arrange an inspection and submit the electronic Notice of
Termination. The Town contacted Dan Sams after the reminder that the fee was late (2/5/2021). The Town
did not contact the Stormwater Program about the Wilmington Regional Office not responding until the
Notice of Deficiency was sent 15 days past the fee due date.

We are writing in response to receipt of the attached Notice of Deficiency relative to Certificate of Coverage
NCC200338. In fact, this will be the second time we have written regarding. Our first email correspondence,
an electronic copy of which is attached, was sent on February 5th in response to notification that the payment
for the 2021 annual fee was overdue. This initial correspondence advised that the project for which the
certificate was issued had been completed. It also reported that we had tried to complete the online form
closing out the project but were unsuccessful in doing so because we could not upload an electronic copy of the
final inspection because to our knowledge, no such inspection had been conducted. As of this writing, that still
seems to be the case. Accordingly, we ask for your assistance in facilitating such inspection.
Your cooperation in this matter is greatly appreciated.
