In response to permit requirements, we have collected an effluent discharge sample from the
septic tank/ sand filter system and have analyzed it for several parameters. The results are as
follows:

Two adults and two children live in this residence. We estimate flow @ 25 gpd/ person.
The original laboratory report and chain -of -custody form is maintained at the Edwards'
residence.
The next effluent sample will be collected and analyzed in 3rd Quarter 2022. In the meantime, if
