On May 17 2012 the Division of Water Quality (DWQ) received your application dated May 10 2012 to fill or
otherwise impact 0 04 acre of 404 wetlands to widen Old Crews Road The DWQ has determined that your
applications were incomplete and/or provided inaccurate information as discussed below The DWQ will require
additional information in order to process your application to impact streams and wetlands on the subject property
This project is on hold and )Incomplete until we receive the additional information requested below If we do not
receive the requested information your project will be formally returned as incomplete Please provide the
following information so that we may continue to review your project
Additional Information Requested
1) It appears that the feature proposed to be impacted as part of the project is depicted as a stream on both the
Soil Survey of Wake County and the USGS topographic maps Therefore this stream would be subject to the
buffer rules Has DWQ conducted a buffer determination on this feature9 If so please provide
documentation of the riparian buffer determination If not please contact DWQ to make a buffer
determination or include buffer impacts on the PCN application
2) The application was signed by the Spangler Environmental Inc who is working on behalf of Bass Nixon and
Kennedy Inc who is working on behalf of the NCDOT/Town of Knightdale Therefore NCDOT or the
Town of Knightdale either needs to sign the PCN form or complete an agent authorization form giving
Spangler Environmental Inc or Bass Nixon and Kennedy Inc the authority to sign the PCN application
