Per Step 1 of the "Six Step Process" for signing up for Stormwater eDMR, I am sending this email on behalf of Kimberly-
Clark Berkeley Mill-Nonwovens. Please see below for a few changes needed to the Owner Verification Form and Permit
Summary Report.

• Owner Verification Form
o Remove "e" from Smythe in the owner/permittee address.
o Change facility address from "Smythe Mills" to "32 Smyth Avenue"
o Change zip in facility address to 28792
Permit Summary Form
o Change facility address from "Smythe Mills" to "32 Smyth Avenue"
o Change zip in facility address to 28792
o Remove "e" from Smythe in the owner address.
o Please replace Michael Baldwin with Raymond Johnson as the owner contact, facility contact, and billing
contact. (I will send a separate email to Aziza Selkane regarding the billing contact, per the instructions)
o The form only lists 1 outfall. The facility has 3 storm water outfalls. What information will you need for
these?
The facility will also be submitting the Stormwater eReporting Registration Form soon (Step 2).
