Subject: Pretreatment Annual Report for the City of Burlington
Dear Mr. Montebello,
Enclosed please find copies of the Pretreatment Annual Report for the City of Burlington. The contents
cover both the South Burlington Wastewater Treatment Plant(NC0023876)and the East Burlington
Wastewater Treatment Plant(NC0023868)for the reporting period of January 1,2020 to December 31,
2020.
Included for all significant industrial users are a brief narrative and an Industrial Data Summary Form.
Required reports include the Pretreatment Performance Summary report, Significant Non-Compliance
report,and an Allocation Table for both wastewater treatment plants. Please note that the Allocation
Tables used are from the Burlington's 2016 Headworks Analyses. Burlington's 2020 Headworks
Analyses were submitted March 17,2020 and are awaiting approval.
I have personally examined and am familiar with the information submitted in these documents and
attachments. Based upon my inquiry of those individuals immediately responsible for obtaining the
information reported therein,I believe that the submitted information is true,accurate,and complete. I am
aware that there are significant penalties for submitting false information.

2020 Pretreatment Annual Report
Narrative Section
The City of Burlington is current with reports as follows:
➢ Allocation Tables are current as of January 3,2020 for East Burlington WWTP
and June 8,2020 for South Burlington WWTP. The Allocation Tables used are
from the 2016 Headworks Analyses.The new Headworks Analyses were
submitted March 17,2020 and are awaiting approval.
➢ Long Term Monitoring Report is current and ongoing.
➢ Headworks Analyses for East Burlington WWTP and South Burlington WWTP
were last approved February 2,2016 and the next due date was April 1,2020.
The Headworks Analyses were submitted March 17,2020 and are awaiting
approval.
➢ The Sewer Use Ordinance was deemed adequate by the PERCS Unit of the
Division of Water Quality March 1,2013 and was adopted by the City of
Burlington City Council November 19,2013.
➢ The Enforcement Response Plan(ERP)was deemed adequate by the PERCS
Unit of the Division of Water Quality February 5,2020.
➢ Industrial Waste Surveys for East Burlington WWTP and South Burlington
WWTP were approved September 19,2016 and the next due date is July 02,
2021.
The City of Burlington started the year with 14 permitted Significant Industrial Users, 2 being
Categorical, and ended the year with 12 permitted Significant Industrial Users, 2 being
Categorical.
The following is a brief discussion of the significant industrial users permitted by the City of
Burlington's pretreatment section. A discussion of permit holders is grouped by East Burlington
WWTP dischargers(NPDES Permit#NC0023868)followed by South Burlington WWTP
dischargers(NPDES Permit#NC0023876),with permit holders arranged from lowest to highest
permit number.
East Burlington WWTP NPDES # NC0023868
Carolina Dyeing and Finishing,LLC,220 Elmira Street,Burlington(IUP#0003)
• No longer in operation and permit dropped as of January 17, 2020.
ITG Burlington Industries LLC-BHF Plant,906 Anthony Street,Burlington(IUP#0006)
• The IUP for this industry was renewed January 01,2017.
• Semi-annual monitoring by the City of Burlington and industry is completed.
• Not in SNC,not on an Order, no construction activities.
Shawmut Park Avenue,LLC, 1821 North Park Avenue,Glen Raven(IUP#0008)
• The IUP for this industry was renewed January 01, 2017.
• Semi-annual monitoring by the City of Burlington and industry is completed.

• This industry was in SNC for Copper and Zinc the 2'6-month period of 2019
and got back into compliance for the 1st 6-month period of 2020. The following
NOVs,penalty,and Public Notice were mentioned in the Narrative Section of
City of Burlington's 2019 PAR but are included in the 2020 Pretreatment
Performance Summary(PPS) sheet:
o NOV for BOD issued 1/21/2020 for running sample out of holding time.
o NOV for temperature issued 1/21/2020 for not sampling twice for
temperature.
o NOV for Copper issued 1/21/2020 for sampling events on 12/26/2020
and 12/27/2020.
o NOV for Lead issued 1/21/2020 for sampling event on 12/26/2020.
o NOV for Zinc issued 1/21/2020 for sampling events on 12/26/2020 and
12/27/2020.
o $245 penalty issued 1/21/2020.
o A Public Notice was posted in the largest local newspaper, The Times-
News,on February 7,2020.A copy of the notice is attached.
• Notice of Violation for Chromium issued 3/2/2020 for sampling event occurring
on 1/23/2020. Back in compliance 3/26/2020.
• Notice of Violation for Copper issued 3/2/2020 for sampling event occurring on
1/23/2020. Back in compliance 3/26/2020.
• Notice of Violation for Lead issued 3/2/2020 for sampling event occurring on
1/23/2020. Back in compliance 3/26/2020.
• Notice of Violation for Zinc issued 3/2/2020 for sampling event occurring on
1/23/2020. Back in compliance 3/26/2020.

• Semi-annual monitoring by the City of Burlington and industry is completed.
• Due to an elevated level of non-target organic compounds present in the samples,
the lab was unable to achieve lower detection limits for semi-volatile organic
analysis for sampling events occurring on 3/20/20, 6/1/20, 6/2/20,7/14/20,
7/23/20,and 12/8/20.A letter from the analyzing lab is included for each date.

sample we were unable to achieve lower detection limits for volatile and semi-volatile
organic analysis. This sample required x500 dilution for semi-volatiles and x20 dilution
for volatiles.

Subject: Progress of the Fats, Oil& Grease Program
➢ The City of Burlington's Fats, Oils and Grease Program was implemented as a
two-phase program December 15, 2000. Phase I consisted of a cooperative effort
relying on the participation of the food establishments.
> Beginning January 01, 2001 all food establishments that have grease traps or
interceptors are required to maintain pumping records on site.
> Identified food establishments must maintain an annual pumping schedule and are
required to keep records that identify who performed the pumping or clean out,
when it was done, and where the waste was disposed.
➢ Inspections for O&G interceptors (grease traps,pits or under counter units) will

> The City of Burlington City Council approved a fee schedule that includes a
$30.00 fee for Fats, Oils and Grease inspections.
> Due to the coronavirus outbreak, in-person inspections were limited to facilities
where high amounts of FOG were seen being discharged. Inspections have been
made at 5 full-service restaurants and 1 industry January 1, 2020 to December 31,
2020. These inspections have addressed employee education in regards to grease
disposal techniques, record keeping, and proper maintenance of grease traps or
interceptors if present. Pumping records were received electronically and
reviewed from 56 food service establishments.
➢ A program for the Alamance-Burlington school system consisting of 17 school
cafeterias is being implemented.
➢ The City of Burlington's efforts to reduce residential Fats, Oils, and Grease into
its sewer system consist of educational information inserts added to customer's
water bills and information being published in the City's quarterly publication
"City Works".
2020 Dental Amalgam Rule Update
According to the NC Dental Board, in 2019,43 licensed dentists are in the City of
Burlington's service area. Some of the dentists on this list have dental facilities that are located
outside the City of Burlington's service area, closed, or do not place or remove dental amalgam.
One-time Compliance Report forms were sent to 31 dental facilities and have been completed by
and received from 23 dental facilities.

Dear Mr. Patterson:
On August 26,2020 a Pretreatment Audit Inspection (PAI)was performed by Jim Gonsiewski of
the Winston-Salem Regional office. Ms. Kristy King, industrial pretreatment specialist for the
City of Burlington,was present. The purpose of this inspection was to determine the effectiveness
of the pretreatment program, which included: a detailed file review, review of the POTW plant
performance, sampling, and compliance judgement program, and adherence to the enforcement
response plan (ERP). Additionally, an inspection of the Indulor facility was conducted in
conjunction with an inspection by the City of Burlington.
Background
The City has twelve (12) Significant Industrial Users (SIUs), two (2) of which are categorical.
Both treatment plants are permitted for 12 MGD. Two SIUs have been dropped from the program.
Lemco changed processes and no longer discharges to the POTW. Carolina Dye and Finishing
closed in January 2020.
Pretreatment Program Element Review
The Headworks Analysis (HWA) renewal was submitted on March 17, 2020. No approval has
been received by the POTW.
The last Industrial Waste Survey(IWS)was submitted on August 2,2016. The IWS was approved
on September 19,2016. The next IWS is due on July 2,2021.
The Sewer Use Ordinance (SUO)was approved on March 1,2013.

The Long Term Monitoring Plan (LTMP) was approved on January 27, 2006 and is being
conducted at the proper locations and frequencies.

A review of the file for Alamance Foods (#0067) revealed that the monitoring data was well
organized and compliant.
A review of the file for Carolina Hosiery (#0055) revealed that the monitoring data was well
organized and compliant.
A review of the file for Kayser-Roth(#0052)revealed that the monitoring data was well organized
and compliant.
The slug control plan for Alamance Foods (#0067)was in good order.
