In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must b submitted to the Division of Environmental Management accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate, The approval of the request will be considered on its merits and may or may not be approved: . In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. e sludge generated from these treatment facilities must be disposed of in accordance with General Statute 1 3- 1 .1 and in a manner approved by the North Carolina Division vision of Environmental Management. 3 7, The issuance of this pennit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. ,. The facilities shall be effectively, maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. . Diversion or bypassing of untreated wastewater from the treatment facilities is prohibited. 10Freeboard in the settling ponds and holding pond shall not be less than two feet at an time. 11. .,any monitoring (inclining any groundwatermonitoring) deemed necessary by the Division of Environmental Management ement o insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed.. 12. Adequate inspection, maintenance, and cleaning shall be provided by the Perrnittee to insure proper operation of the subject facilities, 13, The Permittee, or his designee shall inspect the wastewater recycle facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead t the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a basin or tank; the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. . Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by- pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. d. Any time that self -monitoring information indicates that the facility is not in compliance with its permit limitations. Persons reporting such occurrences by telephone shall also file a written report in letter form within. 15 clays following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. 19. - Upon classification of the facility by the Certification Commission, the P'ermittee shall employ a certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission, 0. This has been done by making one unit to have a. two -compartment oil rarer -off, and the third unit being the tapered > 1 x 41 inspection area. This has resulted in one hole being dug to accept the coil separator and 'the city inspection manhole. The standard separator is designed for mounting at surface level, and the flow of the waste from the station has to be considered in this area. Once the location of the separator has been determined, we use a ba khoe and dig one hole with two depths, one for the deeper depth of the oil sear tor, which is 42 ". The end of the separator, where the inspection area is located is sh arter at 4,1.- after the bole has been dug in these two depths, a bottom layer of 4 to 6 inches of sand helps in the setting of the oil separator, and in being able to keep the separator on a level position. Once the separator is installed into the round, clean water must be used to fill up the first compartment of the oil separator, and be allowed to continue running, until it comes out into the inspection. area. This is a.; must to insure"that we have bbar ed the system, without allowing any mixed product to enter into the system. This also serves as ballast in the event of heave rains before the installation is complete. After the oil separator has been installed, and the unit has been filled with, fresh water, the adjustable oil draw -cuff funnels should be adjusted for your water flow. if the above'ste'ps are not taken and the wash bay would be tied into an empty separator, mixed product would thereby transfer from the first cdmpartment, to the second compartment, and into the downstream side, both the mixture of water and petroleum content ADJUSTING THE OIL DRAW-OFFLEVELS When grater is at a dead standstill in the separator, w have determined d what is called the static eater line. After we h.ve connected the aril separator,; any water outlets that will be going into the discharge line e should at this time be turned on. Again, any and all water boas that will feed into this ,separator should be turned on. In watching the water as it enters the separator, we will see the water -line raise, as it comes into the separator. When it raises to its higheU height* we have established at this point the operating eater line. After the operating water line is established, we can then lower the oil draw off funnels to within l ', to -.. 1/411 above the operating water line.
