The North Carolina Division of Water Resources conducted an inspection of the 144 White Oak Road on
February 16, 2021. This inspection was conducted to verify that the facility is operating in compliance with the
conditions and limitations specified in NPDES WW Permit No. NCG550412. A summary of the findings and
comments noted during the inspection are provided in the enclosed copy of the inspection report.

The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Asheville
Regional Office. The following violation(s) were noted during the inspection:

Ownership form.
_________________________________________________________________________________________________________________________________________________________________________ Septic Tank Septic tank maintenance records were not available at the time of the inspection. NCG550000 Part I. A. 3. Permit Conditions (Operations & Maintenance) Septic tanks shall be inspected at least yearly to determine if solids must be removed or if other maintenance is necessary. Septic tanks shall be pumped out every five years or when the solids level is found to be more than 1/3 of the liquid depth in any compartment.

Effluent Pipe The effluent pipe was broken and where the pipe exits the
treatment system could not be located.

NCG550000 Part I. A. 4. (Operations & Maintenance) All system components,
including but not necessarily limited to, septic tanks, surface sand filters, other
filter components, pump/recirculation tanks, disinfection units and the outfalls
shall be maintained at all times and in good operating order. _________________________________________________________________________________________________________________________________________________________________________ Effluent Sampling Annual sampling records were not available at the time of the inspection.

Remedial actions should have already been taken to correct this problem and prevent further occurrences in the
future. The Division of Water Resources may pursue enforcement action for this and any additional violations of
State law.

To prevent further action, please respond in writing to this office within 30 days upon your receipt of this Notice
of Violation regarding your plans or measures to be taken to address the indicated violations and other identified
issues, if applicable.

Lauren Armeni and Stephanie Williams with the Asheville Regional Office conducted a compliance
evaluation inspection of the single-family wastewater disposal system that services 144 White Oak
Road. This inspection was conducted to determine whether the system is being operated and
maintained in compliance with General NPDES Permit No. NCG550000.
We have been unable to contact the current owner, Mr. Ronald Kitchen, since the last inspection in
August 2016. Mr. Kitchen is still not the listed permittee, and a Change of Ownership form needs to be
filled out and submitted to Raleigh. Another form is being provided with the report.
The vegetation surrounding the home and leading down to the effluent pipe was overgrown, and we
were unable to locate the exact location of the septic tank and sand filter trench. Additionally, we were
unable to locate where the effluent pipe exited the sand filter trench. No septic tank or annual
sampling records were provided because we could not get ahold of Mr. Kitchen.

We were unable to get ahold of the current owner (Ronald Kitchen). Mr. Kitchen has still
not completed and submitted the Change of Ownership form.

Does the facility analyze process control parameters, for ex: MLSS, MCRT, Settleable
Solids, pH, DO, Sludge Judge, and other that are applicable?
The house sits on a rocky bank above Jonathan's Creek. Vegetation was overgrown, and
we were unable to locate the septic tank and sand filter trench.

The general location of the effluent pipe was located; however, the PVC pipe was
completely broken in two pieces and it was unable to be determined where the effluent pipe
exits the sand filter trench. The area around the effluent pipe was overgrown.
