The Division of Water Quality (DWQ) has performed a preliminary review of a 401 Water Quality Certification
application submitted for the above referenced property Additional information will be required in order to
complete the review process
Please provide the following information so that we may continue to review your project
Additional Information Requested
1 Please provide pictures of the shore line bank indicated to be in need of repair
2 Riprap placement will be required as described in the US Army Corps of Engineers General
(Regional) Permit 198200030
Placement of nprap should be limited to only that which is necessary to protect existing/new
bulkheads In general nprap will be placed at the base of all bulkheads and will extend a
maxium of three (3) feet waterward on a 2 1 slope However in case of steep slopes a maxium
of six (6) feet waterward on a 2 1 slope is authorized provided it complies with the applicable
shoreline management program All other fill material will be confined landward of bulkheads
Your plan does not indicate percent slope of the banks to be corrected please plan your project
accordingly

Please explain and provide a brief narrative for the following items How do you plan to
minimize and avoid any surface water impacts during this project such as turbidity sediment to
the Lake etc Provide a timeline that includes a start date for your project timing of the Lake
Level withdrawal if applicable and construction sequence Provide a construction detail of how
the field stone will be placed Note that hydraulic cements and concrete are not allowed to be in
contact with water and a typical design practice is to use filter fabric behind the wall placement
to prevent erosion
Please provide your response in writing to the addresses below by June 15th Four copies of the required
information should be provided to the Wetlands Buffers Stormwater Compliance and Permitting Unit at
