On February 3 2011 the Division of Water Quality (DWQ) issued a 401 Water Quality
Certification to impact 0 35 acre of 404 Wetlands and 219 linear feet of perennial stream in
order to construct the Brightwater Medical Complex in Harnett County Condition 6 of this
certification requires that this project adhere to 401 Stormwater Management
Requirements
On March 26 2012 the DWQ received a stormwater management plan (dated March 23
2012) for the addition of the Brightwater Health Sciences Building on the Brightwater
Medical Complex campus On May 14 2012 the DWQ received additional stormwater
information (dated May 14 2012)
The DWQ approves the SMP as satisfying Condition 6 of the General Water Quality
Certillcalion This approval Is for the purpose and design that you desc abed in your
application If you change your project you must notify us and you may be required to
r,end ! �s a new SMP This sapprovol requires you to follow the conditions listed in the General
Water Quality Certification for the project and the following additional conditions listed
below
1 The SMP approved by the DWQ consists of one wet detention pond and all
associated . -10rYr✓ ✓ate con ✓eyarces inlet and outle+ structures and the grading
and drainage patterns depicted on plan dated March 23 2012 The approved SMP
must be maintained in perpetu►ry No cnanges to ine siruc lural stormwater practices

provide protection of the structures from potential alterations by future property
owners
2 Operation and Maintenance of BMP Maintenance activities for the proposed sand
filter dry detention basin and level spreader vegetated filter strip shall be performed
in accordance with the notarized O&M agreements signed by Mr Johnston
Tilghman (Harnett Forward Together Committee Inc Chairman) on May 9 2012 The
O & M agreement must transfer with the sale of the land or transfer of
ownership /responsibility for the BMP facility DWQ must be notified promptly of every
£transfer
