REQUEST FOR MORE INFORMATION
Dear Sir or Madam:
On March 8, 2007, the Division of Water Quality (DWQ) received your "Courtesy Copy"application
dated May 2, 2007, to fill or otherwise impact 15 linear feet of perennial stream and 145 linear feet of
intermittent stream to construct the proposed residential subdivision. The DWQ has determined that your
application was incomplete and/or provided inaccurate information as discussed below. The DWQ will
require additional information in order to process your application to impact protected wetlands and/or
streams on the subject property. Therefore, unless we receive five copies of the additional information
requested below, we will have to move toward denial of your application as required by 15A NCAC 2H
.0506 and will place this project on hold as incomplete until we receive this additional information.
Please provide the following information so that we may continue to review your project.
Additional Information Requested:
1. Please resubmit your application using the attached, most current, 2005 PCN application form.
2. Please show all stream impacts including all fill slopes, dissipaters, and bank stabilization on the
site plan at a scale of no smaller than I "=50'.
3. Please indicate proposed lot layout as overlays on the site plan.
4. Please provide cross section details showing the provisions for aquatic life passage.
5. Please provide documentation that DWQ or Division of Forest Resources has made
intermittent/perennial stream determinations on the site or contact DWQ to schedule
intermittent/perennial stream determinations on the site.
6. Please locate any planned sewer lines on the site plan.
7. Please specify the percent of project imperviousness area based on estimated built-out conditions
and provide an inventory of all impervious surfaces (building footprints, roads, driveways, decks,
sidewalks, gravel-covered areas, etc.).

8. If the project imperviousness area referenced in item No. 7 above is 30 percent or greater, please
provide the location of, and detail for, any proposed stormwater management practices from the
entire park as required by the General Certification for Nationwide Permit 39.
9. Attached is a copy of the General Certification 3631 (NW 29). If your project does not meet all
the conditions which allow it to be issued without written concurrence from the DWQ, it will be
necessary for you to formally apply with DWQ, by submitting five complete application
packages, and the appropriate fee for review of your project and written concurrence from the
DWQ.
Please contact the DWQ within three weeks of the date of this letter to verify that you have received this
letter and to provide us with a date that we may expect to receive this requested information. If we do not
receive this requested information within one week of the date you indicate it will be forthcoming, your
project will be withdrawn and you will need to reapply with a new application and a new fee. Please
contact me in writing and Lauren Cobb of the DWQ Raleigh Regional Office. If we do not hear from you
within three weeks, we will assume that you no longer want to pursue this project and we will consider
the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
authorized (at this time) by the DWQ. Please call Ms. Cyndi Karoly or Mr. Ian McMill at 919-733-
