Pursuant to Title 15A of the North Carolina Administrative Code 02H .0117 and therein, as provided in
North Carolina General Statute 143-215.3(a)(2), the Division of Water Resources (DWR) is requiring the
submittal of monitoring data and information related to the subject permitted wastewater system. This
request is necessary to determine compliance with the subject permit and other applicable state
regulations. Specifically, DWR is requiring submittal of the following information:
1. All analytical results and related information (e.g., sampling location, time, purpose, field
measurements, chain -of -custody sheets, sampler identification, etc.) generated during the
month of March 2018 for the wastewater system. This shall include, but not necessarily be
limited to, effluent and process control sampling and measurements data, all related lab bench
sheets, and the results of samples collected to monitor any changes in influent quality. This
request includes all analytical parameters monitored, field and laboratory generated results;

2. All Operator and Back-up Operator in Responsible Charge operational logs and notes. Please
also include laboratory documentation related to observations, measurements, and
adjustments in operations related to the wastewater system operation; and,
3. Please provide a narrative and timeline of all mill production outages occurring during March
2018. Please include all information (source, characterization, volume, cause, etc.) related to
any changes in the influent waste stream(s) to the treatment system occurring during this time.
The required information is to be received by this office no later than September 2gth. Please contact us
