On April 11 2012 the Division of Water Quality (DWQ) received your application dated April 11 2012 to
fill or otherwise impact 1 508 square feet of Zone 1 Neuse River protected riparian buffer and 929 square feet
of Zone 2 Neuse River protected riparian buffer for a residential subdivision The DWQ has determined that
your application was incomplete and /or provided inaccurate information as discussed below The DWQ will
require additional information in order to process your application to impact streams and wetlands on the
subject property This project is on hold and incomplete until we receive the additional information
requested below If we do not receive the requested information your project will be formally returned as
incomplete Please provide the following information so that we may continue to review your project
Additional Information Requested
1 Please provide correct topographic contours between the base of the level spreader lips (for all
three proposed level spreaders) and the top of bank for the receiving stream
2 As discussed between John Harris and Annette Lucas on May 14 2012 the level spreader design
will be further updated to the engineered filter strip design which will greatly reduce the length
of the level lip that is needed Please insure that the level spreader design complies with Chapter
8 of the North Carolina Stormwater BMP Manual available at
http //portal ncdenr org/web/wq/ws/su/bmp manual Please submit an updated LS VFS
Supplement Form for each of the level spreaders with all required items that required updating
