On March 1 2012 the Division of Water Quality (DWQ) received your application dated March 1 2012 to
fill or otherwise impact 0 015 acre of 404 wetland 122 linear feet of stream channel (permanent impacts) to
construct a roadway for a subdivision On April 13 2012 the DWQ received additional information that was
not complete The DWQ has determined that your applications and additional information was incomplete
and /or provided inaccurate information as discussed below The DWQ will require additional information in
order to process your application to impact streams and wetlands on the subject property This project is on
hold and incomplete until we receive the additional information requested below If we do not receive the
requested information your project will be formally returned as incomplete Please provide the following
information so that we may continue to review your project

1 Please provide all of the required items that were omitted from the April 13 2012 submittal
including plan sheets PP 1 through PP 5 and plan sheets D1 and D2 The DWQ cannot review the
project until these items are received
Please submit three copies of this information within 30 calendar days of the date of this letter Please send

NC 27699 -1650 If we do not receive this requested information within 30 calendar days of the date of this
