Water Quality Classification of nearest receiving water: WS -III 2c. River basin: Cape Fear River Basin 3. Project Description 3a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: The proposed work will include the extension of an existing culvert crossing of an intermittent stream. The Par Drive road crossing is within the Sandy Creek Golf Course Residential Community. The 18 -hole golf course and it's associated infrastructure was begun in the early 1990's. Although originally planned for an entire large -lot residential community only one home exists on this parcel. Sourrounding properties are forested, in agriculture or are residential. 3b. List the total estimated acreage of all existing wetlands on the property: +/-5 3c. List the total estimated linear feet of all existing streams (intermittent and perennial) on the property: There are approximately 6,780 linear feet of stream on the property. 3d. Explain the purpose of the proposed project: A culverted crossing currently exists in the location of the proposed impacts however the road crossing is not paved and has steep slopes that do not comply with current NC Dept. of Transportation standards. This crossing was likely installed in the early to mid 1990's. In order to meet current subdivision roadway standard widths additional culvert length and fill is required. 3e. Describe the overall project in detail, including the type of equipment to be used: The project will involve the placement of cement culvert pipe extension pieces on both the inlet and outlet side of the existing culvert as well as the placement of a dissipation pad on the outlet side of the culvert to reduce high - velocity energy and reduce downstream errosion. Heavy machinery such as backhoes will be used to complete this work. 4. Jurisdictional Determinations 4a. If yes, explain in detail according to "help file" instructions. * * *A search for previously issued approvals and permits was conducted by Mr. John Thomas of the USACE however no file was found for this property. Based on a review of aerial photography it apprears that the site was developed in the early 1990's at a time when strream impacts were measured in square footage instead of linear footage and written approvals were not required for such impacts as those that occurred on the property. 6. Future Project Plans 6a. Is this a phased project? ® Yes ❑ No 6b. If yes, explain. No additional permits or impacts are proposed to occur with regards to the Par Drive roadway. As stated previously the residential portion of the Sandy Creek Golf Club has not been completed and additional permitting would likely be required should there be an unexpected demand for such housing in this region.
