3a Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this
application The site is currently in early successional vegetative cover in the uplands The wetland areas are in mixed
pine hardwood forest cover The majority of the property in the immediate vicinity of the project is in residential use with
some commercial properties Phase I of this development was previously completed and includes internal roadway
infrastructure sanitary sewer lines and existing homesites The sewer line that was requested in the original application
for Phase I has also been installed

3d Explain the purpose of the proposed project The purpose of the proposed work is to construct a road crossing to provide
access to Phase II This road crossing was previously approved on 08/25/2010 and has since been redesigned to reduce
overall permanent stream and wetland impacts The sewer crossing that was previously approved in the original
application (2009) for Phase I of Lakedale was installed There are three additional sewer crossings to connect to
existing sewer lines /manholes that are also being requested within this application in order to complete the rest of the
development and provide sanitary sewer service to those phases

The overall project will involve the construction of a residential subdivision using heavy equipment typical to grading and
land development work The project is to be constructed at the location of the former Lakedale Mill industrial facility to

Phase I of the Lakedale development included the construction of a sanitary sewer line to service the western portion of
the project Phase I includes 45 lots on the west side of the wetland /perennial stream that bisects the project No
impacts other that the sewer connection were required to develop Phase I which has been completed The Action ID #
for Phase I is 200900284 The application was dated October 27 2009 The 404 permit is dated 1/15/2010 (see
attached copy) In 2010 a PCN was submitted for the proposed road crossing of a perennial stream/wetland
complex to provide access for Phase II of the Lakedale development The 404 Approval was received on
08/25/2010 (AID 200900284 attached) This application request is a modification of that necessary roadway
crossing where the total stream and wetland impacts have been reduced Additionally 3 sanitary sewer
connections are required (to connect to existing sanitary sewer lines) in order to provide service to the remaining
portions of the development This application request also details those unavoidable impacts as proposed

The applicant proposes to construct and subsequently permit the residential subdivision in phases The final design of
the sewer and roads for phase 3 will be completed after the initial phases are constructed Phase II will involve the
construction of an additional 45 lots east of the wetland /perennial stream that bisects the projects The impacts as part of
Phase II will be the construction of a road across the wetland /perennial stream and additional sanitary sewer connections
in the northern portion of the project It should also be noted that this is a modification request of previously approved
impacts for the proposed roadway crossing the applicant and design team were able to successfully reduce the overall
proposed stream and wetland impacts for this road crossing The purpose for phasing the project is to construct homes
at a pace that will meet demand as well as generate funding to progress with the future phases

Specifically describe measures taken to avoid or minimize the proposed impacts in designing project
The applicant requested a detailed delineation of streams and wetlands prior to site plan development in order to
determine the extent and location of streams and wetlands on the subject property The site plan has limited
permanent impacts to one roadway crossing on a perennial /important stream Streams are not crossed multiple
times by roads As previously explained in the original application package this road crossing is necessary to access
the eastern portion of the property which is separated from the western portion (Phase 1) by a perennial stream and
wetland system
The applicant is also aware of the regional conditions associated with the Nationwide Permits 29 With respect to
residential development conditions (Regional Conditions section 4 1) the applicant does not anticipate any impacts
to streams or wetlands associated with stormwater facilities there are no proposed single family recreation
facilities and there are no permanent wetland or stream fills proposed within the floodway and the stream
classification for this project is C (i a not Outstanding Resource Waters or High Quality Waters)
