provided inaccurate information as discussed below

1 DWQ requires five (5) application sets and the appropriate fee to begin the review and approval
process Visit www ncwaterquality org under Surface Water Wetlands and Stormwater to read
more about the 401 approval process and fee structure
2 A Public Notice issued by the US Army Corps of Engineers will be necessary to begin the review
and approval process Please note that the following must be received prior to issuance of a 401
Water Quality Certification
3 DWQ is unable to review of this project since Hi Tech Recycling Inc is not listed on the NC
