We do have NCEEP receipts and documents that reference the 2006 permits (attached). Explain the purpose of the proposed project:* This lot had 0.5 acres of wetland fill impacts permitted in 2006 (2006-41141-148). The same permit included 0.04 acres of fill for Lot 58 in the same subdivision. That permit was issued to the owner at the time (Ann Warner). The properties were filled per the permit in 2006 and mitigation from NCEEP (currently NCDMS) was satisfied. At the time, mitigation credits were only available in 0.25 acre increments, so that full amount of credits were purchased by the previous owner (NCEEP receipt and documentation attached). The current owners (the Folliards) have house plans and are ready to build. In order to accommodate the on -site septic, house footprint and driveway, they would need to impact an additional 400 sq.ft. (0.009 acres) of wetlands. Total impacts for the past and present permit combined would be less than 0.1 acres total. 41. IWWS-98-1—R3 IMPORTANT NOTICE TO OWNER FROM INNOVATIVE APPROVAL - The Puraflo Peat Biofilter is a pre—engineered proprietary treatment system. The containment modules are made of pre —assembled, UV —protected polyethylene material, with approximate dimensions of 7.08 feet long by 4.58 feet wide by 2.5 feet deep. The modules are fitted with UV —protected, removable polyethylene lids, which contain ventilation and drain holes. Each module in the system is installed at the exact same elevation, with effluent flow uniformly split between the separate modules. The fibrous peat treatment media is residues of Eriophorum (Cotton Grass) plants, extracted from raised bog peats (other natural residues and peat fines are also present). The estimated life of the peat media Is currently 15 years. The media may need to be replaced, in part or in full, In order to maintain specified treatment standards (see note below) TREATMENT PERFORMANCE STANDARDS (TS-1): Tertiary treatment without nitrogen reduction. The Puraflo Peat Biofilter shall be designed, installed and operated to meet the following standards: 1. Biochemical Oxygen Demand, 5—day (BOD5) < 15 mg/L 2. Total Suspended Solids (TSS) < 15 mg/L 3. Ammonium —nitrogen (NH4—N) <10 mg/L 4. Fecal coliform bacteria densities < 10,000 colonies/100 ml (MPN) Standards are arithmetic means, except fecal coliforms is a geometric mean. System classification, management and inspection shall be in accordance with Rule .1961. Puraflo Peat Biofilters shall be classified at a minimum as a Type Va system according to Table V(a) of Rule .1961(b). System Inspections: Both the local health department and the operator —in —Responsible Charge (ORC) must conduct monitoring inspections of Puraflo Peat Biofilter Systems at a minimum frequency as specified in Table V of Rule .1961(b) and the Operation Permit. THIS SYSTEM REQUIRES REGULAR INSPECTIONS BY THE DARE COUNTY HEALTH DEPARTMENT AND THE OPERATOR —IN RESPONSIBLE CHARGE ORC . THE OWNER IS REQUIRED TO CONTRACT WITH A QUALIFIED ORC AND WILL B RESPONSIBLE FOR FEES ASSOCIATED WITH THE CONTRACT AS WELL AS FEES ASSOCIATED WITH ROUTINE WASTEWATER SAMPLING AND TESTING. THE DARE COUNTY HEALTH DEPARTMENT ALSO CHARGES YEARLY INSPECTION FEES. IN ADDITION THE OWNER SHOULD BE AWARE THAT THE TREATMENT MEDIA HAS A LIMITED LIFESPAN AND MUST BE REPLACED AS NEEDED TO MAINTAIN THE SPECIFIED TREATMENT STANDARDS. DISCUSSIONS WITH SEVERAL LOCAL OPERATORS AND INSPECTORS INDICATE THE LIFE SPAN OF THE PEAT IS SUBSTANTIALLY LESS THAN THE 15 YEARS NOTED IN THE INNOVATIVE APPROVAL.
