On March 1 2012 the Division of Water Quality (DWQ) received your application dated March 1 2012 to fill or
otherwise impact 0 015 acre of 404 wetland 122 linear feet of stream channel (permanent impacts) to construct a
roadway for a subdivision The DWQ has determined that your applications were incomplete and/or provided
inaccurate information as discussed below The DWQ will require additional information in order to process your
application to impact streams and wetlands on the subject property This protect is on hold and incomplete until we
receive the additional information requested below If we do not receive the requested information your project
will be formally returned as incomplete Please provide the following information so that we may continue to
review your project
Addittional Information Requested
1) Please provide cross section details showing the provisions for aquatic life passage Please show headwalls
for all culverts (current information only shows detail on southern end of Area 1 culvert)
2) It appears that based on the impervious surface ( <24 %) and lack of piped conveyance that the project may be
considered low density per the requirements of GC 3820 For each vegetated stormwater conveyance please
provide a completed Low Density Supplemental Form along with all the supporting information (see
http //portal ncdenr org/web/wq/sw/su/bmp manual) If this development does not meet the impervious
surface thresholds and/or utilizes piped conveyance please provide a stormwater management plan to DWQ
meeting high density development requirements
