The ditch along the east property line will be blocked with an earthen dam to prevent runoff from the Phase I street from entering the pond. 2. The pond depth will be reestablished at 6.5' , the slopes will be mowed, and eroded areas of the slopes will be repaired and reseeded. As I was going through the file after the meeting, I noticed that a revised application and fee for Sand Ridge had not been submitted. The Divsion must have a signed application and a fee for the modification to Sand Ridge prior to issuing the permit for Brithaven or Sand Ridge. Please submit the application and fee as soon as possible. The supplement form and calculations have already been submitted. A preliminary review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: Per NCAC 2H.1003 (e), the signature of the consulting engineer can be accepted on the application only if accompanied by a signed letter of authorization from the owner. 2. The calculations for required surface area have been done separately. The drainage and impervious areas for Britthaven and Sand Ridge should be added together to make the calculations. Doing them separately generally results in a larger surface area and volume than are needed. 3. Please note that the Town of Newport has alleged that drainage changes have been made such that Lots 1-12 now drain to the detention pond. The revised permit for Sand Ridge issued on October 20, 1995, did not include these lots as part of the drainage area to the pond. I had requested that Mr. Stallings address this issue in a September 18, 1996, compliance inspection report, but have received no answer to date. For purposes of pond sizing, I have included all lots covered by the original permit. 4. The plans you submitted do not show the planted islands in the cul-de-sacs, and have a revision date of 6/24/96. The original approved plans indicated that there would be planted islands in the cul-de- sacs, and are dated 10/19/95. I will request that Sand Ridge submit a modification to their current permit to account for this change, and to modify the overall subdivision approval due to the selling of a portion of the permitted site area, and the reduction in the permitted surface area (see #7 on the following page). 5. Please complete the offsite supplement form, enclosed, for Britthaven. The wet pond supplement you submitted will need to be revised to include all of the drainage area to the pond, not just what Britthaven contributes. The pond has been certified by a design professional who is not qualified to design detention ponds. Please have a registered NC Professional Engineer provide the certification. Please include verification of the presence of the 2.75" small diameter orifice to provide .the required 2-5 day drawdown. The certification submitted by the surveyor only refers to a water, level that is currently 12" below the riser opening. Please verify the actual elevations of the permanent pool and storage pool. 7. The surveyor has indicated that 17,510 square feet of surface area has been provided at the permanent pool elevation, but you still use 18,000 in the supplement and in the calculations. Unless Mr. Stallings is planning to excavate the pond to provide the additional 490 square feet, please use the 17,510 figure in the calculations and in the supplement. 8. The impervious area breakdown does not match up with mine. This is partly due to the fact that I deleted the 31,200 square foot pond area as impervious. The runoff generated within the pond area is considered negligible. If the cul-de-sacs in Sand Ridge no longer have planted islands, additional pavement must be accounted for. If the drainage for Lots 1-20 has been altered to flow into the pond, additional impervious area must also be accounted for. Please check and verify the cul-de-sac design, and the drainage routing for Lots 1 20. A modification to Sand Ridge must be submitted by Mr. Stallings to account for these discrepancies. 9. The agreement between MAR-STAL and Britthaven makes no mention of maintenance responsibility or compensation made to MAR-STAL for maintaining the pond. Since MAR-STAL is currently responsible for maintenance of the pond, the permit for Britthaven will be written to reflect this. Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to December 23, 1998, or the application will be returned as incomplete. Per 15A NCAC 2H.1003(h) (the stormwater rules), applications for permit renewals shall be submitted 180 days prior to the expiration of a permit and must be accompanied by a processing fee, which is currently set at $505.00. If this is still an active project please complete and submit the enclosed renewal application prior to June 21, 2012. If this project has not been constructed and a permit is no longer needed, please submit a request to have the permit rescinded. If you have sold `the project please provide the name, mailing address and phone number of the person or entity that is now responsible for this permit. Enclosed is -a form for change of ownership, which should be completed and submitted if the property has changed hands. Your permit requires that upon completion of construction and prior to operation of the permitted stormwater treatment system, a certification of completion be submitted to the Division from an appropriate designer for the type of system installed. This law impacts any development approval issued by the Division of Water Quality under Article 21.of Chapter 143 of the General Statutes, which is current and valid at any point between January 1, 2008, and December 31, 2010. The law extends the effective period of any stormwater permit that is set to expire during this time frame to three (3) years from its current expiration date. On August 2, 2010, the Governor signed Session Law 2010- 177 which grants an extra year for a total of four (4) years extension. Accordingly, Stormwater Permit # SW8 931006 which has already expired on December 21, 2008, is now in effect until December 21, 2012. Please resubmit the renewal application and fee at least 180 days prior to December 21, 2012, which would be June 21, 2012. The project is in Newport Townshlp, Carteret County, North Carolina and is being developed by Leonard & Lana Farrugia. The work proposed will Include catch basins, storm drains and a new Swale to drain into the existing on site storm drainage system. 5ee flan. Land use in the vicinity le residential. All adjoining properties are residential subdivisions, and will not be, affected by the work. planned Erosion and Sedimentation Control Practices A temporary gravel construction entrance/exit will be, installed at the north project entrance. During wet weather it maybe .necessary to wash vehicle tires at this location. The entrance will be. graded so that runoff water will be directed away from proposed roadway. Ae soon ae final grade is reached on the proposed roadway, the subgrade will be stabilized with a 6-inch course of NC DOT standard size ABC Stone to prevent erosion and dust during the construction prior to paving. 51opes will be, lightly roughened by disking just prior to vegetating. 5urface stabilization will be accomplished With vegetation and mulch ae specified in the vegetation plan. The stormwater collection system described will provide the following methods for control of sediments and for removal of pollutants from the developed site runoff. -1- MAINTENANCE PLAN Stormwater Pond The Owner shall maintain the pond during the construction phase of the project and after construction is completed for all routine and non -routine maintenance tasks. The Owner shall also be responsible for the perpetual Care of the stormwat.,er retention pond. All erosion and sediment control practices shall be, checked for stability and proper operation following every runofF producing rainfall and In no case less than once every week during construction. All repairs shall be made immediately and shall be, in accordance with the approved Erosion and Sedimentation Control Plan. The stormwater retention pond shall be Inspected during the construction phase of the project following every runoff producing rainfall and in no case less than once a week. Check the embankment, spillway, outlet, and Swale for erosion damage, and inspect the embankment also for piping and settlement. Make all necessary repairs Immediately. iZemove all trash and other debris from around the decant orifice structure and from around the pool area. Periodic inspection of the discharge orifice shall assure that the system is not clogged. The retention pond shall be, Inspected for a build-up of sediment. If and when the sediment layer exceeds 1& inches thick on the average above the design grade, then this sediment shall be, removed. At the close of the construction phase of the project, the pond shall be cleaned of excess sediment to the designed grade and begin functioning as stormwater retention pond. All regrading or repairing of the side slopes, Seeding, mowing and any and all repairs shall be performed prlor to close-out of the construction phase. The grass -lined Swale from pond overflow structure shall be properly inspected and maintained against erosion, weeds, and woody vegetation. All seeded areas shall be malntained to sustain the required vegetation. Any and all bare areas shall be replanted as necessary, with all repairs made immediately. Owner shall be. responsible for all routine and non -routine requirements associated with the maintenance of the stormwater retention pond. Upon completion of earth disturbing activIVIco and for 12 months following completion of the work, the owner shall inspect the site following every runoff producing rainfall. A written log shall be maintained of any needed repairs noted at the inspection, the name of the Contractor completing the repairs, and the date such repairs are completed. 1.Miowlma The side -slopes, embankment, buffer area, areas within the fence, areas adjacent to the emergency spillways and the grass -lined swalce shall be mowed at least once a month during the main growing season and at least two other times during the year to prevent woody growth and to control weeds. Grass shall be, mowed to a "meadow -like" condition and shall be maintained according to Section 6.11 Permanent Seeding ofthe approved Erosion and Sedimentatlon Control Plan. -5- 2, IntipP ionti. The stormwater retention pond shall be inspected (at a minimum of) on an annLzal basis to ensure that the structure operatee in the manner originally intended. Additional inspections shall be conducted during wet weather, or times of extreme weather to determine if each pond is functioning properly. Inspection priorities shall include (but not limited to) the following: checking the embankment for any erosion, subeldence, cracking, bare areas in the grass, and tree growth; the condition of the emergency spillway and adjacent ditch; the fence and adjacent planting buffers; the accumulation of sediment; clogging of the decant orifice Structure; the Adequacy of upstream and downstream channel erosion protection measures including any modifications which have occurred to the contributing watershed; and the pond structure Including the stability of the side slopes. Corrective measures and repairs shall be done to the pond side elopes, embankment, emergency spillway and grass lined Swale where settlement, slumping, and erosion has occurred. Repair as necessary. Repair or replace the riprap protecting the channel near the outlet as required to stop the erosion. All repairs shall be done in accordance with the original plans and specifications. 5. Nuisance Control. Owner ehall maintain the retention pond and adjacent areas eo as to control the possible probleme of weeds, odors, algae, and insects. Proper maintenance shall take care of most problems except in times of extreme dry weather. Owner shall also use biological control against mosquito's and algae through the use of fish and fat head minnows when possible in lieu of chemical applications. 1, 5trucl'ure Repairs and Replacement. Owner shall replace all deteriorated iniet/outlet works and decant orifice structure when deterioration from use and age causes them to fail and function properly. Owner.ehall be responsible for all costs incurred with their replacement. Replacement shall be in accordance with original plane and specifications. 2. 5ediment Removal, Owner shall be, responsible for the removal of excess accumulated sediment in the pond when the storage capacity is significantly reduced from the design capacity due to the properly designed and functioning stormwater retention pond. Owner Shall be responsible for all incurred costs of removal and proper disposal of the sediment in accordance with all local, state, and federal regulations. 5ediment removal/clean out cycle for etormwater retention pond is recommended on a ten to twenty year cycle.
