The Division of Water Quality (DWQ) received the Individual Permit project material on March 14 2012
for the above referenced project This is a change from being originally submitted as a NW42 A Public
Notice issued by the US Army Corps of Engineers will be necessary to begin the review and approval
process Please note that the following must be received prior to issuance of a 401 Water Quality
Certification
Additional Information Requested
Additional Application Fee The fee for applications is now $240 for projects impacting less than
an acre of wetland and less than 150 linear feet of streams (whether intermittent or perennial)
For projects impacting one or more acres of wetland or 150 linear feet of streams (whether
intermittent or perennial) the fee is $570 DWQ received check # 1014 for $240 on January 20
2012 with the original submission The open water impacts show as 2 44 acres thereby
necessitating the $570 fee Please remit a check for the balance of $330 made payable to NC
DWQ
