I conducted a compliance evaluation inspection (CEI) at the subject project site in Winston Salem Forsyth County North
Carolina on March 20 2012 My findings are detailed below
The following observations were noted during the DWQ inspection
DWQ# 11 0829 The temporary crossing authorized under by this certification had been restored to natural
conditions as required You provided a signed completion form to me during the inspection
2 NCGO 10000 Rain Gauge (Part I, Section B, No 1) A rain gauge was observed on site
3 NCGO 10000 Rain Gauge Data (Part I, Section B, No 1) Rain gauge data records are being kept as required
4 NCG010000 Minimum Monitoring and Record Keeping (Part I Section B No 3) Records of the inspection
maintenance and repair of erosion and sedimentation control (E &SC) measures and stormwater discharge outfalls
(SDO) are being kept
You are reminded that all E &SC measures and SDOs must be inspected and the inspections must be recorded at least
once every seven calendar days and after every rainfall event of greater than 0 5 inches E &SC measures must still
be inspected at least once during those weeks when there is no rainfall Additionally this office is to be notified
immediately in the event of sediment release from the site into any waters of the State Please also note that the
NCG010000 permit remains open and in full effect until the NC Division of Land Quality deems the site fully
stabilized and closes their permit for the site
