On February 22 2012 the Division of Water Quality (DWQ) received your application dated February 15 2012 to
fill or otherwise impact 180 linear feet of stream channel (permanent impacts) 30 linear feet of stream channel
(temporary impacts) 6 770 square feet of Zone 1 Neuse River basin protected riparian buffer and 980 square feet of
Zone 2 Neuse River protected riparian buffer to construct a temporary access corridor and stream bank stabilization
The DWQ has determined that your applications were incomplete and /or provided inaccurate information as
discussed below The DWQ will require additional information in order to process your applications to impact
streams wetlands and buffers on the subject property This project is on hold and incomplete until we receive the
additional information requested below If we do not receive the requested information your project will be
formally returned as incomplete Please provide the following information so that we may continue to review your
project

1) Please re submit your site plans on full plan sheets at a scale of no smaller than 1 =50 with topographic
contours shown
2) In Section 3e Project Description of the PCN
The text states that Because the in stream portion of construction will require only about 2 days it will
be conducted without dewatering the stream Please note that the Water Quality Certification for
Stream Bank Stabilization requires Work In The Dry A site visit by DWQ is requested before an
exception to this condition can be considered for this project Please contact Natalie Landry at 919

erosion and sediment control measures are shown on the plans Text in paragraph two refers to erosion
control measures on the plans Please clarify In addition paragraph three states that the path for
backhoe access into the stream will be left bare until the south bank work is completed and the backhoe
removed then this final stream bank area will be planted Please note that this area must be stabilized
within 7 days of work temporarily or permanently ceasing Show the location of any stockpiles on the
plans
3) Please describe the reason for the additional 60 linear feet of stream bank disturbance noted on PCN Page 4
of 11 paragraph 5 (avoidance and minimization)
4) Please describe how the stream downstream of the project activity will be protected during the restoration
activity
5) In the PCN attachments the section Construction Survey references Drawings that will be provided to the
Contractor Please provide a copy of these to DWQ In the same section the text also references `the
centerline of new stream Please clarify
Please submit three copies of this information within 30 calendar days of the date of this letter Please send
additional information to DWQ Central Office, Attn Karen Higgins, 1650 Mail Service Center, Raleigh,
NC 27699 -1650 If we do not receive this requested information within 30 calendar days of the date of this
letter your project will be withdrawn and you will need to reapply with a new application and a new fee If you
need additional time to compile the information requested above please email Natalie Landry
