Dear Mr. Earp:
On January 14, 2021, Blane Houck from the Winston-Salem Regional Office of the Division of
Energy, Mineral, and Land Resources (DEMLR), conducted a compliance inspection for the
Synergy Recycling LLC facility located at 320 S Gibson Dr., Madison, North Carolina. A copy of
the Compliance Inspection Report is enclosed for your review. The inspection and file review
revealed that the subject facility is excluded from NPDES stormwater permit coverage through a
No Exposure Certification. The receiving waters for this facility are designated as Reed Creek, a
Class IV waterbody in the Roanoke River Basin.
As a result of the inspection, the following deficiency is noted:
1) Failure to Maintain Conditions of No Exposure Certification
This facility has not adequately maintained conditions required by the No Exposure
Certification. Refer to the enclosed compliance inspection report for more information.
Required Response
Submit an action plan to address site maintenance and clean-up within 30 calendar days of receipt
of this notice. The action plan must provide a date by which the maintenance and clean-up will be
completed.
Thank you for your attention to this matter. This office requires that the deficiency, as noted above
and detailed in the enclosed inspection report, be properly resolved. Failure to address the

Inspection Summary:
This site was inspected for compliance on January 14, 2021. Megan Tabb, with Synergy Recycling, was present during the
inspection. At the time of inspection, the following items were noted:
1. It was stated that self -re -certification was not conducted for the years 2019 and 2020. No self -re -certification documents
could be located on -site.
2. The external siding of the building was observedto be in disrepair, such that electronics debris were discharging from the
building.
3. The outside parking area, weigh station, and loading dock were observed to be littered with electronics debris.
4. A mulch stockpile and scrap metal were observed in the outside parking lot area.
