40 In accordance with your application received May 29, 1987, we are forwarding
herewith Permit No. 15154, dated September_ 21, 1987, to the Carolina Friends School,
for the construction and operation of the subject non -discharge type waste treatment
and disposal system.
This permit shall be effective from the date of issuance until September 30,
1992, and shall be subject to the conditions and li_mtati_ons as specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjud.icator.y hearing upon
written request: within 30 days following receipt of this permit. This request must be
in the form of a written petition, conforming to Chapter- 150B of North Carolina
General. Statutes, and filed with the Office of Administrative Honr:i.ngs, Post Office
Drawer 11666, Rnlei_gh, North Carolina 27604.
One (1) set of approved plans and specifications is being forwarded to you. If

Statutes of North Carolina as amended, and other applicable Taws, Regulations Rules
PERMISSION IS HEREBY GRANTED TO

FOR THE
construction and operation of a 4000 GPD non -discharge type wastewater treatment and
disposal. system consisting of an existing 4000 GPD wastewater treatment plant, a 66
GPM pump station with dual pumps and high water alarm, two-2000 gallon capacity pump
• tanks, a tablet chlorinator, approximately 800 feet of 2.5 inch force main, a 55,903
gallon capacity storage lagoon, a 90 GPM pump station with one pump and high water
alarm, a 34,636 square feet irrigation field, and all associated valves, piping and
appurtenances to serve Carolina Friends School, pursuant to the application received
May 29, 1987, and in conformity with the project plan, specifications, and other
supporting data subsequently filed and approved by the Department of Natural
Resources and Community Development and considered a part of this permit.
This permit shall be effective from the date of :issuance until September. 30,
1.992, and shall be subject to the following specified conditions and :Limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at. all times.
4. This permit is not transferable.
5. Upon the availability of a municipal or regional. sewerage collection system, the
subject wastewater treatment system shall be abandoned, and all wastewater
discharged into the municipal or regional sewerage system.

6. The wastewater treatment and disposal facilities shall. be effectively maintained
and operated at all times so that there is no discharge to the surface waters,
nor any contamination of groundwaters which will render them unsatisfactory for
normal use. In event the facilities fail to perform satisfactorily, including
the creation of nuisance conditions, or failure of the irrigation area
to adequately absorb the wastewater, the Perm:ittee shall take such immediate
corrective action as may be .required by the Division of Environmental. Management.
7. Any monitoring deemed necessary by the Division of Environmental Management to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed.
8. Monitoring wells shall be constructed and sampling schedules arranged as
required by the Division of Environmental Management and constructed in
accordance with 15 NCAC 2C .0100.
9. The sludge generated form these treatment facilities must be disposed of in
accordance with General Statute 143-215.1 and in a manner appr.ovable by the North
Carolina Division of Environmental Management.
10. A suitable year round vegetative cover shall. be maintained.
11. Dikes shnl.l be constructed so as to prevent surface water. runoff from the spray
field.
1.2. The Raleigh Regional Office, telephone number 919/733-2314, shall be notified in
. advance of operation and backfilling of the installed wastewater treatment and
disposal system so that an in place inspection can be made. Such notification
to the Regional Supervisor shall be made during the normal office hours from 8:00
a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.
13. Wastewater shall not be applied during inclement weather on until twenty-four. (24)
hours following a rainfall event of 1/2-inch or greater in twenty-four. (24) hours
or when the ground is frozen and in a condition which would result in surface
runoff.
1.4. Diversion or bypassing of the untreated wastewater from the treatment
and disposal facilities is prohibited.
15. The Permittee, at least six (6) months prior to the expiration of this Permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein, and as indicated, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
16. The application rates shall not exceed:
a. 52 inches/year
b. 0.25 inch/hr
17. The issuance of this permit shall not relieve Carolina Friends School of the
responsibility for damages to surface or groundwater.s resulting from the
operation. of this facility.

18. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of
this facility.
19. No type of wastewater other than that from Carolina Friends School shall be
sprayed onto the irrigation area.
20. No traffic or equipment shall be allowed on the disposal area except while
installation occurs or while normal. maintenance .is being performed.
21. Adequate inspection, maintenance, and cleaning shall be provided by the
permittee to insure proper operation of the subject fac Ui.ti.es.
22. The Permittee is responsible for the operation and maintenance of the entire
treatment and disposal system; including, but not limited to, the following
items:
a. The septic tank shall be checked every six (6) months and pumped out as
required, but no less frequently than once per year..
b. The tablet chlorinator shall be checked every week until the actual usage
rate is established; and thence, on a monthly basis. Tablets shall be
added so as to provide proper chlorination.
G. The storage and pump system shall. be inspected monthly. The scum layer
shall be removed at the same interval as the septic tank is cleaned out.
. The alarm system shall be checked. The pump operation shall be checked.
d. The spray system shall be inspected monthly to make certain of the sprayers
proper operation, that the vegetative growth allows proper spray patterns,
that the soil is absorbing the disposed treated wastewater with no surface
runoff., and that no objectionable odors are being generated.
23. Failure of the Permittee to properly operate this system i.s snbiect to a penalty
up to $10,000 per day.
24. Failing to meet permit conditions or by violation of the State's surface or
ground water regulations will require cessation of operation of the facilities.
25. The Permittee shall employ a certified wastewater treatment plant operator in
responsible charge of the wastewater treatment facilities. Such operator must
hold a certificate of the grade at least equivalent to the classification assigned
to the wastewater treatment facilities by the Ge.r.tif.icati.on Commission,
26. The spray irrigation field must be located no closer. than 1.00 feet from any public
or private water supply well.
27. The storage lagoon and spray irrigaiton areas shall be labeled as such and fenced.
28. No animals should be allowed to graze on the irrigation si.i:e during the irrigation
of wastewater.

29. An annual report shall be submitted to the Raleigh Regional Office by January 15,
of each year providing, but not limited to, the following information:
a. dates of irrigation
b. volume of wastewater irrigated
C. continous monthly and year-to-date hydraulic loading (inches per acre)
30. Prior to beginning wastewater disposal operations, two (2) monitor wells, one (1)
upgradient and one (1) downgradient, must be installed to monitor groundwater
quality. The location and construction details for these wells must be approved
by the Raleigh Regional Office, from which a well, construction permit must be
obtained.
31. The monitor wells must be sampled initially after construction (and prior to
waste disposal operations) and thereafter every March, July and November for the
following parameters:
Nitrate pH
Total Ammonia Total Organic Carbon
Total Dissolved Solids Chloride
Water Levels Total Organic Halides (in November only)
Total Coliforms
The measurement of water level. must be made prior to sampling for the remaining
parameters.
The analytical methods used for Total Organic Halides must be capable of
detecting the total of all halogenated organic compounds present at a
concentration of 5.0 parts per. billion (ppb) or greater.. Any individual
halogenated organic compound present at a concentration equal. to or above the
method detection limit (MDL) must be .identified, quantified, and reported with
sampling results.
The results of the sampling and analysis shall be sent to the N.C. Division of
Environmental Management on Form GW-59 [Compliance Monitoring Report Forml.
32. Any additional groundwater quality monitoring, as deemed necessary by the
Department, shall be provided.
