The Division of Water Quality issued a high density Stormwater Management Permit, Number SW8
951101 to Wrightsville Park, LLC for the Hawthorne Place project on December 15, 1995. This
permit expired on December 15, 2005. A letter was sent to you on July 11, 2007 requesting that an
application for renewing the permit be submitted and, at this time, a response has not yet been
received. Per 15A NCAC 2H.1003(h) (the stormwater rules), it is your responsibility to submit an
application to renew the permit 180 days prior to the expiration of a permit.
If this is still an active project please complete and submit the enclosed renewal application along
with the processing fee, which is currently set at $505.00 prior to June 3, 2013. If this project has not
been constructed and a permit is no longer needed, please submit a request to have the permit
rescinded. If you have sold the project please provide the name, mailing address and phone number
of the person or entity that is now responsible for this permit. Enclosed is a form for change of
ownership, which should be completed and submitted if the property has changed hands.
Your permit requires that upon completion of construction and prior to operation of the permitted
stormwater treatment system, a certification of completion be submitted to the Division from an
appropriate designer for the type of system installed. This is to certify that the permitted facility has
been installed in accordance with the permit, the approved plans, specifications and supporting
documentation. if you have not already provided a Designer's Certification to our office, please
include a copy with your permit renewal request and processing fee. A copy of the certification form
is enclosed for your convenience.
You should be aware that failure to provide the Designer's Certification and the operation of a'
stormwater treatment facility without a valid permit, are violations of NC General Statute 143-215.1
and may result in appropriate enforcement action including the assessment of civil penalties of up to
$25,000 per day.
If a renewal application is not submitted prior to June 3, 2013, enforcement actions may be initiated,
which may include the assessment of civil penalties. If you have any questions, please feel free to

Dear Mr. Albanese:
The Division of Water Quality issued a Coastal Stormwater Management Permit, Number SW8951101
to Wrightsville Park, LLC for a High Density pond to serve Hawthorne Place on 12/15/1995. This permit
expired on 12/15/2005. Section .1003(h) of 15 A NCAC 2H .1000 (the stormwater rules) requires that
applications for permit renewals shall be submitted 180 days prior to the expiration of a permit and must
be accompanied by a processing fee, which is currently set at $420.00. If this is still an active project
please complete and submit the enclosed renewal application in a timely manner. If this project has not
been constructed and a permit is no longer needed, please submit a request to have the permit
rescinded. If you have sold the project, or are no longer the permittee, please provide the name, mailing
address and phone number of the person or entity that is now responsible for this permit. Enclosed is a
form for change of ownership, which should be completed and submitted if the property has changed
hands.

November 16, 1995 correspondence we offer the following with
respect to your comments:
1) We generally do not include building dimensions on, our plan
but we have enclosed separate sheets with that information.
2) Roof drainage notation is included on revised plans.
3) We have included a forebay in the pond and the outf all pipe
has been revised to discharge at the opposite end of the
pond.
4) Pond dimensions have been added.

The Wilmington Regional Office received a Stormwater Management Permit Application for
Hawthorne Place on November 1, 1995. A preliminary review of that information has determined that
the application is not complete. The following information is needed to continue the stormwater review:
1. Please provide a dimensioned footprint of the building on the plans. Are the steps/decks of

✓ 2. Please note on the plans that roof drainage must be directed into the parking area.
t/3. Please design a forebay at the pond inlet pipe, and angle the inlet pipe more toward the
skinny arm of the pond. The proposed design may create a dead zone where no treatment
of runoff will occur.
